Who We Serve
Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

About our Program:

CSU: Is a 23hr 59min unit for youth placed on a 5150 hold that require Intensive stabilization. The goal is to divert youth from more extended psychiatric hospital stays.

JOB SUMMARY

Works within the vision, mission and philosophy of the agency. Creates a strength- based relationship with youth and assures a positive environment. Assists in the design and implementation of interventions and strategies in support of the youth and family’s goals. Ensures the provision of safety and health in a variety of environments for youth and families in accordance with agency standards. Coaches primary care givers and other community members.

RESPONSIBILITIES AND DUTIES

1. Provides supervision and living environment support for youth in residential/crisis stabilization unit settings.

2. Builds solid, cooperative, culturally responsive relationships with youth and families as well as community resources to assist with the planning, organizing, implementation and evaluation of appropriate activities to achieve family identified goals.

3. Builds on individual strengths, concerns, and needs with balanced focus on the family as a unit to assist in the implementation and achievement of goals and outcomes.

4. Works with youth and family teams to support family driven, strength-based planning and interventions.

5. Conducts observations regarding specific target behavior, track progress and modifies plans with the family to support their intended identified outcome.

6. Identifies behavioral triggers and suggests environmental modifications to support positive behavioral change

7. Provides targeted mental health services and crisis stabilization interventions consistent with service plans, documents in alignment with MediCal regulations and achieves set productivity expectations.

8. Provides and documents other non-billable services and activities if required by program.

9. Ensures the safety of and is accountable for the whereabouts of youth at all times.

10. Understands and takes advantage of therapeutic opportunities in crisis situations.

11. Monitors effectiveness and efficiency of behavioral and crisis management interventions with youth.

12. Participates in physical restraints using Therapeutic Crisis Intervention techniques.

13. Physical interactions with children of variable ages such as but not limited to games, sports, and other activities.

14. Documents interactions, practices, and maintains administrative expectations in a timely manner. Provides direct billable mental health services, documents in alignment with MediCal regulations, and achieves set productivity expectations.

15. Plan strategic and effective daily structures activities that give youth opportunity to work on goals state in their individualize treatment plans.

16. Communicate effectively with youth, parents, team members, supervisors, and outside stakeholders such as school personnel, and to remain neutral, always acting as a representative for youth and their families.

17. Utilize effective interpersonal skills promote positive and cooperative relationships with youth and their family members through the therapeutic treatment process.

RESIDENTIAL ONLY DUTIES

1. Works in the community directly with youth and families to develop and implement safety and skill refinement plans.

2. Supports families in bridging to and or building on natural resources and accessing community based program supports that will continue to support over time.

3. Develops resources within the community to serve children and families.

4. Participating in family finding team activities by identifying family finding need, obtaining authorizations, creating teams, conducting searches and setting deadlines. Completes Connectedness Maps as needed.

5. Facilitates and/or co-facilitates, supports, and model participation in healthy group dynamics within various settings including family homes, schools, parks, and recreation centers and treatment facilities. Provides an open forum for expression of feelings and ideas when appropriate.

6. Driving to appointments with clients and their families or other key contacts.

MINIMUM EDUCATION AND/OR EXPERIENCE

High School Diploma or General Education Development (GED) required

Preferred Qualifications

Bachelors (BA/BS) in a related field preferred

One (1) year experience working with children preferred.

OTHER SPECIFIC REQUIREMENTS:

• Pass an initial physical abilities test and be certified in crisis intervention techniques within the first 30 days and annually.

• Employee shall be at least 21 years of age or by program requirement.
