Job Summary
Reporting to the Mgr-PACE RN, this position is responsible for the direct social work case management services to the participants of the PACE program. Provides such social services support as participant screening, case management, counseling and referral.
Accountabilities
- Participates in the Interdisciplinary Team's initial assessments, care planning and periodic reassessments (minimally every 6 months) of participant's case. - 30%
- Coordinates 24-hour care delivery. Provides individual and family counseling, develops and leads group counseling activities. - 30%
- Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants. - 10%
- Encourages effective relationships among staff geared to team building and maintenance of a cohesive team. - 10%
- Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community. - 10%
- Participates in Program and Policy development of Social Work component. - 10%
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Masters Degree in Social Work
1 year experience working with the frail or elderly
Required Certifications, Registrations, Licenses
LMSW (Licensed Masters Social Worker) or LISW (Licensed Independent Social Worker)
Required Knowledge, Skills, Abilities
- Basic computer skills
- Knowledge of office equipment (fax/copier)
- Proficient computer skills (word processing)
Other Qualifications or Experience
Driving, if an essential function of position, is determined by leadership and designated Facility assignment. Driving or home visits may/may not be required based on Facility and role assignment. Incumbents in positions with driving designated as an essential function, are subject to an MVR review and Prisma's eligibility requirements for driving.
