Responsibilities of a Hospice Social Worker - LMSW:

  • Visit patients and families in their homes to assess their needs and provide appropriate resources and support
  • Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans
  • Provide counseling and emotional support to patients and their families, helping them cope with the challenges of terminal illness
  • Connect patients and families with community resources and services to address their needs and improve their quality of life
  • Advocate for patients and families to ensure their needs are met and their voices are heard
  • Maintain accurate and up-to-date documentation of patient interactions and interventions
