Job Description

JOB SUMMARY

Responsible for wide-range of Social Work Programs, which include, but are not limited to Adult Protective Services [APS] and Adult Services [AS] programs serving elderly and disabled clients, as well as Child Protective Services [CPS], Foster Care/Adoption, crisis intervention, emergency outreach, intake, and on-call services.

ESSENTIAL JOB FUNCTIONS
  1. Responsible for Adult Protective Services and Child Protective Services investigations and dispositions, as well as follow-up services, as needed
  2. Provides case management, which includes home visits with adults, families, children, and collaborating with other professional services to include schools, medical providers, substance abuse services, mental health and other service providers
  3. Responsible for assessing and addressing individuals' emotional, social, and physical needs
  4. Responds to abuse and neglect complaints and conducts Child Protective Services and Adult Protective Services investigations and assessments
  5. Conducts assessments and interviews with victims and perpetrators of abuse and neglect
  6. Responsible for covering intake duties for CPS/APS reports and emergency assistance requests
  7. Conducts Assisted Living and Nursing Home screenings including completing the Uniform Assessment Instruments with Public Health Nurses
  8. Handles intakes, develops service plans, and address self-sufficiency needs
  9. Conducts home visits to assess and monitor service delivery, provide intervention, and monitor safety issues
  10. Provides transportation and accompaniment to appointments with clients
  11. Facilitates and supervises visits with parents and their children
  12. Monitors and ensures adoption assistance cases are in compliance, sending annual affidavits for completion, coordinating services for the needs of the child
  13. Responsible for Emergency Guardianship and Guardianship cases, including monitoring, reviewing, and filing of Guardianship reports
  14. Represents Department in court for protective orders, guardianship hearings, and other child or adult related matters before the court
  15. Facilitates trainings and represents the Department on various committees
  16. Ensures cases comply with local, state and federal regulations
  17. Documents and maintains case files pertaining to all service programs
  18. Enters and maintains accurate data in the State information systems for the various programs
  19. Handles data input and other reporting pertinent to the program areas
  20. Completes required local and State monthly reports
  21. Completes purchase orders and submits invoices for payment
  22. Assumes emergency duties including shelter duty, in the event of a disaster, and serves on a rotating duty roster for the duration of an event
  23. Handles after hours on-call duties for CPS, APS, and Police/Fire/EMS services in the community on a rotation
  24. Completes the mandated trainings through Virginia Department of Social Services
  25. Performs other related job duties as assigned
QUALIFICATIONS

Education and Experience:
  1. Bachelor's Degree or Master's Degree in Social Work
  2. Experience in human services and/or previous experience in a social services
Special Qualifications:
  1. Live within a 30-minute drive time from the City to respond to on-call duties
  2. Valid Virginia Driver's License
Preferred Qualifications:
  1. Two years of experience working in the human services field
  2. Master's degree in Social Work or related field
Knowledge, Skills and Abilities:
  1. Knowledge of casework principles, assessments, and networking with both internal and external resources
  2. Knowledge of federal, state, and local social services programs
  3. Knowledge of current trends in social work and human services
  4. Ability to communicate effectively with individuals and groups, both orally and in writing, particularly with those with disabilities
  5. Ability to interview, assess needs, counsel and refer individuals to resources as needed
  6. Ability to develop and implement services plans
  7. Ability to make sound judgements within the framework of existing laws and policies
  8. Ability to establish and maintain collaborative and effective professional relationships with internal and external customers and colleagues
  9. Ability to be available for on-call services on a rotating basis
  10. Ability to be available for emergency shelter and/or community service duty, in the event that the Emergency Operations Center [EOC] is activated, or for crisis intervention services
  11. Ability to operate small office equipment, including copy machines or multi-line telephone systems
  12. Ability to operate computers for data entry, word processing and/or accounting purposes

PHYSICAL DEMANDS

The work requires the ability to exert moderate but not constant physical effort to perform light work, typically involving some combination of grasping, handling, hearing, kneeling, lifting, mental acuity, pulling, pushing, reaching, repetitive motion, speaking, standing, stooping, talking, visual acuity, walking and may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (up to 20 pounds), visual perception and discrimination, the ability to perceive and discriminate sounds, perceive and discriminate color, perceive odors, and require oral communications.

WORK ENVIRONMENT

Work is regularly performed in an environment where the employee must deal with crisis situations that require major decisions involving people, resources, and property.
