The Intensive Case Manager - CDC:

  • Assist in the development and implementation of an outreach plan to enroll persons living with HIV/AIDS into the case management program. Implement specified behavioral interventions as outlined by funding sources.
  • Conduct HIV Rapid Testing and pre and post-test counseling at EPIC locations or in the community.
  • Assess the client’s needs and personal support systems, develop an individualized needs assessment and service plan, coordinate services to implement the plan, monitor the efficacy of the plan and periodically re-evaluate the plan as related to clients’ on-going needs. Link client to community resources.
  • Empower client with support, education and community resources to access the services needed to fulfill the needs identified on client’s needs assessment. In addition, advocate for the client within all available resources to obtain services to meet client needs.
  • Participate in community events to raise awareness of HIV, PrEP and how to reduce HIV infection risk
  • Serve as primary point of contact of individuals interested in PrEP that were contacted during in-reach, off-site outreach, and onsite case management activities.
  • Assist in scheduling appointments for PrEP consults and work with providers and medical staff to establish care.
  • Maintain all program documentation and client records in order to ensure accurate project data.
  • Promote, articulate, and model the vision, mission and values of the grant project.
  • Must be able to work within the project team setup to provide client crisis intervention, staff coverage and be willing to share responsibilities as designated.
  • Accurate reporting of scheduled time worked and activities performed.
  • Collect all data required for program evaluation.
  • Effectively communicate progress to supervisor and/or other persons designated by supervisor.
  • Other related duties as assigned.
Qualifications and Requirements:
  • Education and/or Experience: Bachelor’s degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
  • Current driver’s license, automobile insurance and accessibility to a vehicle
  • Certifications (i.e. CPR): 500/501 HIV Testing and Training, Rapid Testing Certification, Hepatitis C and other STI certifications, or the ability to obtain within six months of hire.
  • Flexibility to work an alternative work schedule that includes nights and weekends.
  • Flexibility to attend meetings and trainings which may fall outside the regularly assigned work hours.
  • Excellent communication skills and ability for detail
  • Experience at working both independently and in a team oriented collaborative environment.
  • Bi-lingual (Spanish) a plus
  • Sensitivity to cultural differences and alternative sexual orientations to include the Gay, Lesbian, Bi-Sexual and Transgendered Communities.
