Job Description:

  • Certified Targeted Case Manager through Florida Certification Board
  • Complete documentation related to authorizations for Targeted Case Manager (TCM) services
  • Meet with client to open case.
  • Link, Assess, and Advocate on behalf of individuals and families.
  • Develop Assessments, and Individual Service Plans with the input of individual and family.
  • Complete face to face meetings per Medicaid policies and procedures
  • Arrange for transportation for client’s community appointments as needed.
  • Competed billing related documentation
  • Attending staffing on behalf of client with foster care. Schools, to ensure the needs of the client are being met.
  • Possess interpersonal and communication skills that will allow TCM to work with various clients, teams and families.
  • Attend monthly supervisions, and required trainings.

· This is a part-time to full-time contract position

  Flexible work schedule

  Valid Driver's License

· TCM must be Bilingual in English and Spanish

Qualifications:

· Bachelor’s degree with a major in a human services-related field

  • A minimum of one year full-time or equivalent experience working with children and adults.
  • Must have or be willing to get professional liability insurance.
  • Must have or be willing to obtain Professional Liability Insurance.

OR

  • Bachelor's Degree in a non-human services-related field
  • Three years of full-time or equivalent experience working with children and adults
  • Must be Bilingual in English and Spanish
  • Successful completion of AHCA-approved mental health targeted case management training within three months of initially providing Medicaid services
  • Florida Driver’s License
  • Must have or be willing to obtain professional liability insurance.

 

TCM will need to be local as *Some travel is required for this position.*

This is a contracted position, and no benefits are provided.
