Position Summary
  • Under the general supervision of the manager, the care coordinator nurse provides clinically-based care coordination to support the delivery of quality patient care as efficiently as possible. The care coordinator has accountability for individual patients ensuring quality clinical outcomes, and discharge planning for patients within the assigned caseload. The care coordinator is responsible for knowing the patient's "story" and connecting the patient's story to the medical condition and treatment plan. This role will focus on clinical outcomes and evidence based practice as a mechanism to assure quality care. With the patient, family and health care team, the care coordinator creates and organizes a discharge plan appropriate to the patient's needs and resources and ensures that the patient is receiving the right care, at the right level across the continuum.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: BSN required.
  • License/Certifications: Licensed to perform as a Registered Nurse in the State of Maine
  • Experience: Three to five (3-5) years' current acute care clinical bedside experience.
  • Critical thinking skills.
  • Excellent communication skills.
  • Ability to problem solve creatively.
