Job Profile Summary:

Assists patients and their families with personal and environmental stressors which interfere with obtaining maximum benefit from medical care. Social work interventions may include clinical psychosocial assessment and care planning, information and referral, discharge planning, case management, psychoeducation and psychotherapy when needed for adjustment to illness and/or treatment for behavioral problems. Participates in orientation of new social work staff. May participate in supervision of students completing Field Placement. Acts in accordance with the Social Work Code of Ethics.

BASIC PURPOSE OF THE JOB

To create an environment that will provide structure, reduce inappropriate behavior and establish a routine. Modeling for patient appropriate boundaries and healthy expectations. Will provide assistance with de-scalation of patient outburst/inappropriate/violent behavior. Will assist with creating a safe environment. Will also complete assessments and provide support in other areas of the ED as needed.

JOB REQUIREMENTS

Supervisory Responsibilities: No

Minimum Education: Masters Degree in Social Work Required

Degree: Social Work

License/Certification Required: Licensed Masters Social Work Licensure required within one (1) year of hire; Case Management certification desired

Minimum Work Experience: 1 year post-Masters Social Work experience preferred; 1 year as a Case Manager preferred.

DUTIES AND RESPONSIBILITIES
  • Implement structure and routine by developing a schedule for patient to include:
  • waking up times, and meal times
  • Assist with collaboration for schoolwork as needed
  • Organizing video lesson times as part of their schedule
  • To organize Showers/bedtime/personal hygiene schedules
  • Promoting mental health/wellness - for example: sitting at desk to eat meal
  • Implement regular body movement/regulation
  • Setting expectations with parents
    • Visitation expectations (who can visit and requirement of visitation - to include exceptions to visitation rules
    • Work to establish visitation schedule with families while patient is in behavioral health area
    • Provide list of approved activities for families
  • Doing regular check in's with a goal to assess mood/affect every 1-2 hours during waking hours
    • this is NOT a behavioral health check
  • Develop behavior modification plans as appropriate for behavioral concerns
  • Developing an initial internal safety plan for SIB/SI and de-escalation plan for aggression - this would start if patient has been here for 24 hours or longer and parent would contribute to the development of the internal safety plan.
    • Safety plans and de-escalation plans will be shared with staff assigned to patient.
  • These internal safety plans will be identified before a patient becomes combative/aggressive in order to promote regulation.
  • Assist on the Behavioral Health Code Response Team- when this is implemented
  • Expert in CPI
  • Educates the client, family or caregiver, and members of the health care delivery team about treatment options, community resources, insurance benefits, care management, etc. so that timely and informed decisions can be made.
  • Empowers the client to problem-solve by exploring options of care, when available and alternative plans when necessary to achieve desired outcomes.
  • Actively participates in relevant meetings and committees.
  • Collaborates with leaders in program development, supports change and adapts to meet program goals.
  • Seeks new knowledge and participates in learning opportunities to support the care of children with medical complexities and their caregivers.
  • Will also provide psychosocial assessments, mental health assessments and Behavioral health check-ins as needed
  • Will assist with Behavioral Health mental health assessment due to positive ASQ screenings
  • Willingly accepts any other assignment that may be requested.
PHYSICAL REQUIREMENTS
  • Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position.
  • Medium lifting, pushing and pulling is required for 20-50 lbs occasionally, 10-20 pounds frequently and 10 lbs constantly to move objects. Sudden emergency situations have the potential for exposure for lifting or moving of up to 100 lbs. Frequent bending, walking, sitting, squatting, reaching, and standing are required. Keyboard/computer use and/or repetitive motions may be required.
