Description

Service Delivery
  • Ensure services identified in the IPP are effectively delivered by competent, qualified staff. Provide and/or direct service area training as needed.
  • Ensure Homeward Bound's compliance with the Vulnerable Adults Act and Maltreatment of Minors Act. Assist in policy development and implementation, and staff training/coaching.
  • Ensure allocated resources are used appropriately and within identified parameters.
  • Plan, implement, and evaluate the changes needed to continuously improve the services provided.
  • Monitor and ensure the delivery of quality program planning and development consistent with HBI standards, policies and procedures.
  • Monitor and ensure the delivery of personal assistance consistent with HBI standards, policies and procedures.
  • Establish effective, responsive and generalized communication systems which contribute to the continuity of services and supports.
Health & Safety
  • Ensure resources are in place to deliver quality health care consistent with HBI standards, policies and procedures.
  • Monitor infection control.
  • Ensure nutritious, good-tasting meals are served.
  • Ensure purchasing, food handling and preparation is consistent with HBI standards, policies and procedures.
  • Conduct period site audits to evaluate compliance with company and regulatory agency safety standards, policies and procedures.
  • Ensure employee and resident injuries are promptly reported and appropriate measures are taken to ensure the health and safety of employees and residents.
Quality Assurance
  • Be accountable to stakeholders by being accessible, available, and responsive. Develop and maintain positive and professional relationships with all stakeholders including individuals, families, guardians, staff external supports.
  • Conduct formal and informal information gathering activities to assess the satisfaction of stakeholders and service quality. Report results to the Director of Program & Operations.
  • Lead and conduct internal investigations and implement corrections as required. Serve as HBI's liaison to external agencies on investigations. Manage all internal and external VA/MOM reports through to completion.
  • Conduct audits and reviews of site compliance with company policies, procedures and standards including health, nutrition, safety, environmental, and program planning, development and implementation. Report findings and recommendations to the Administrative Leadership Team.
  • Provide on-site monitoring, training and implementation of practice and compliance with the rules and regulations.
  • Conduct regular audits of programming documentation and forms to monitor staff compliance with service delivery requirements.
Leadership & Supervision
  • Role model professional behavior consistent with HBI's Professional Behavior Standards, policies and procedures.
  • Train, mentor and assist Program Management team members in programming, incident reporting, corrections, services quality, and other related areas.
  • Advocate for the people served by and employed by HBI.
  • Facilitate conflicts to resolution.
  • Provide support, direction and recognition of employees.
  • Encourage personal growth, team development and cohesiveness.
  • Recognize limitations of self and team.
  • Provide and/or direct service area training.
  • Create and maintain environments that attract and retain employees.
  • Mentor and develop employees you directly supervise.
  • Conduct timely performance appraisals and reviews.
  • Monitor and manage onboarding, evaluation and termination of employees.
  • Monitor and manage employee work schedules, work rules, work performance, and professional development.
  • Monitor and manage the payroll process including EPL requests, leaves of absence, and overtime control procedures.
  • Assume additional responsibilities as requested and necessary to maintain program and operation integrity.
  • Attend and participate all required meetings and trainings.
Finance, Environment & Property
  • Ensures that HBI obtains the framework rates to which it is entitled and that its staffing patterns match the authorized frameworks.
  • Ensures the submission of applications for and obtains those DWRS "exceptions" that allow for additional revenues and to which HBI may be entitled.
  • Ensures that all approved "exception" applications are renewed annually on a timely basis.
  • Collaborates with HBI's CEO, Director of Program & Operations, Director of Nursing, and Director of Finance & Property as necessary to thoroughly complete an accurate picture of the individual served when supervising the completion of exception applications.
  • Develops effective working relationships with county and state personnel in charge of decision-making over the new system and individual rate development as necessary.
  • (For ICF Only) Tracks the development and implementation of the State of MN's new "case mix" system that will set minimum and maximum budgets for individuals.
  • Monitor current program expenditures and supply costs.
  • Account for budget variances in assigned service area.
  • Provide cost information on proposals.
  • Assist in the development of program budgets for following year.
  • Manage external purchased services, fees and contracts.
  • Ensure systems and standards promote a safe and secure environment.
  • Ensure environmental conditions are consistent with HBI standards in that the environments are safe, clean, free of hazards with appropriately functioning and maintained equipment.
Requirements
  • Must have a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older.
  • Must meet one of the following experience/education criteria:
    • a baccalaureate degree in a field related to human services, AND one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
    • an associate degree in a field related to human services, AND two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
    • a diploma in a field related to human services from an accredited postsecondary institution AND three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
    • a minimum of 50 hours of education and training related to human services and disabilities AND four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
  • Meet the licensing, contractual, and other mandated requirements of the state and county.
  • Valid Minnesota Driver's License with acceptable record required.
