Location - The Apostle Center for Care, 54-bed Skilled Nursing Facility

Position/Schedule:

  • Full Time; 8:00am-4:30pm; Monday-Friday
Qualifications
  • Two years administrative experience, preferred.
  • Proficiency utilizing a PC, ability to effectively operate software programs utilized by the facility, and internet research capabilities.
  • Knowledge/experience of Medical Terminology including ICD-9 Coding.
  • Proficiency in typing and other clerical duties required.
  • Two years Medical Records experience, preferred.
  • Ability to organize and meet deadlines.
  • Knowledge of MDS assessments, and familiarity with Long Term Care regulations.
Essential Job Responsibilities and Objectives:
  1. Develop and maintain Policies and Procedures for Medical Records.
  2. Enter updated medical records information into computer system or physical chart.
  3. Prepare and maintain monthly data sheets and resident charts.
  4. Conduct monthly chart review and perform CQI auditing including physician telephone orders and others as assigned by supervisor. Thin charts. Tabulate findings and communicate status of audits to supervisor.
  5. Manage medical records requests.
  6. Gather and correct placement of all admission information. Prepare chart identification, room labels, and photographs upon admission.
  7. Prepare medical forms, obtain hospital and/or resident medical records from previous physician's office including immunization, recent physician notes, lab work and medication list.
  8. Maintain Admission, bed hold, Admit/discharge tracker
  9. Maintain current knowledge of long-term Care regulations as they relate to record keeping and utilize the Medical Records Consultant to facilitate compliance with regulatory requirements. Notify Director of Nursing of compliance concerns.
  10. Assist in answering incoming telephone calls in a professional manner.
  11. Ensure paperwork for previous month is completed by the 1st week of the current month.
  12. Coordinate records, forms, statements and information for long term care companies monthly
  13. Coordinate resident appointments, transportation, and escorts. Ensure appropriate paperwork is completed for appointment.
