Case Manager - CPRS - Behavioral Health Unit (Trenton, NJ)

Case Manager

The Rescue Mission of Trenton is seeking aCase Manager for our Behavioral Health Unit (BH). Under the direction of the Vocational Development Coordinator, the Behavioral Health Case manager primarily works with BH clients on income development, obtaining identification and medical benefits. What it’s like to work at the Rescue Mission of Trenton Founded in 1915 and located in Trenton New Jersey we serve men and women who have no place else to turn for shelter, clothing, treatment and food. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. Our dedicated staff in the Shelter and our Behavioral Health Unit offer care, support and encouragement to all that reside here. Our clients are assisted by staff with treatment options, medical help, job placement, financial planning, education, counseling and mental health treatment. This position is an important part of helping our clients navigate their independent living upon discharge.

Position Details

  • Meet with all clients to apply for all identification documents with specified timeframe

  • Meet with halfway clients to apply for social services benefits

  • Assist clients in obtaining SSI and SSD Income

  • Transport clients to and from established appointments

  • Collaborate with outside agencies

  • Follow-up with clients and external agencies for securing documents and benefits

  • Documentation of all updates on Master Roster

  • Entering case management notes in Credible System

  • Serve as Liaison for Vocational Team Roles

Qualifications:

  • Certified Peer Recovery Specialist Certification

  • Two years experience in case management

  • Proficient in Microsoft Office Suite and Electronic Health Record

  • Well organized, able to prioritize among competing tasks and able to work independently

  • Fluent in English and Spanish preferred (both written and verbal)

  • Excellent relationship-building/people skills

  • Valid Driver License

Hourly Rate: $18.54 Monday - Friday 8a-4p, plus a comprehensive benefit and training package

Additionally, this position qualifies for the Quarterly Bonus

$1.00 for every hour worked paid out quarterly through June 30, 2025, if you are actively working at the time the additional funds are scheduled for pay out.
