Position Overview:

We are looking for a caring and committed professional to become a part of our organization as a Case Coordinator specializing in Child Welfare. In this capacity, you will deliver comprehensive case management services to our clients and their families, ensuring that our fundamental values are upheld while positively influencing their lives.

Primary Duties:

  • Take on assigned cases and perform timely visits to both children and their guardians.
  • Collaborate with relevant stakeholders to negotiate and formulate case plans.
  • Maintain accurate documentation of case activities and manage physical case files.
  • Process service requests and provide prompt referrals to necessary services.
  • Participate in court proceedings, individual case meetings, and other essential appointments.
  • Carry out child safety evaluations and arrange for emergency placements or services when required.
  • Conduct thorough searches for parents and family members when necessary.
  • Offer pertinent information to potential caregivers and facilitate visits as needed.
  • Ensure all assignments are completed within established timelines.

Requirements:

  • Bachelor's degree in a Human Services discipline; a degree in Social Work is preferred.
  • A minimum of one year of relevant experience is required.
  • Possession of a valid driver's license, capability to transport clients, and readiness to travel as necessary.
  • Ability to adjust to irregular hours, perform light lifting, and be flexible to work on-call as needed.
