Case Manager- Intensive Program

Hope For Youth is seeking to hire an Intensive Case Manager for their Intensive & Supportive Case Management (ISCM) program ISCM is an out-patient preventative service that provides children and their families with the support and treatment needed to stabilize a youth's immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCM's intensive track (ICM), can accommodate the needs of up to 12 children and their families. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County. Through interdisciplinary assessments, DSS and Child Protective Services provide the Hope For Youth ICM team with information pertaining to the families past and current functioning so that Hope For Youth's Intensive Case Managers can provide recommendations for going forward. Communication between Suffolk County and HFY is ongoing.
To be a successful Intensive Case Manager you must willing and able to:
- Providing services to maintain child in current living environment and work to achieve positive outcomes/deliverables as outlined in the service contract/grant.
- Assessing and addressing safety and health concerns in the child's current home environment and provide appropriate advocacy as needed.
- Conducting twice weekly face to face contacts/home/community contacts and completes all necessary reports, paperwork and forms as outlined in the program operations manual.
- Demonstrates flexibility with modifications to program model, including changes to job responsibilities.
- Maintain a caseload up to 4 clients.
- Field work
- Documentation and related paperwork
- Staff meetings, committees, supervision, Interdisciplinary Treatment Team Meetings
- Training and staff development
- Other assignments, as needed
- BSW or Bachelors degree in related field
- Valid NYS Driver's License in good standing
- Access to a reliable vehicle that can be used on a regular basis for home visits and transports throughout Suffolk County.
- Excellent writing skills/time management.
- Ability to be flexible with scheduling (work evenings, possible weekends if necessary).
- On call 24/7 via mobile phone and Internet technologies for around-the-clock communication and to respond to any emergencies.
- Physical Requirements: read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving
This opportunity is being offered at $45K/year at 35 hours per week flexible
