Our services include a state-of-the-art psychiatric urgent care center and a soon-to-be-built pediatric behavioral health center, outpatient, inpatient, partial hospitalization (day programs), assessment and testing, residential, addiction treatment and specialty services such as crisis response, employee assistance programs, forensic psychiatry and psychology, and neuromodulation. On-the-job educational programs for nurses, psychologists, advanced practice providers, psychiatrists, and chaplains are equipping the next generation of care providers.

Position Summary:

The Transition Clinical Coordinator has the general responsibility of coordinating care and triage of clients to the most appropriate level of care. Duties performed include but are not limited to referring for determination of admission to PHP, handling insurance information, gathering relevant information for clinicians, maintaining patient information, discharge planning and coordination of aftercare. Transition Clinical Coordinator must demonstrate excellent customer service and have enthusiasm about the program and the Pine Rest organization.

Principal Duties and Responsibilities:
  • Ability to work collaboratively with other community agencies on behalf of clients and/or the organization.
  • Communicate and serve as a liason with other involved clinicians, organizations, and families, to ensure continuity of care, coordination of services and excellent patient experience.
  • Conduct face-to-face and telephone consultation with patients to assess their clinical condition and to recommend the PHP level of clinical care.
  • Coordinate admissions and/or intakes, including interfacing with insurance and managed care companies for authorization.
  • Ensures that medical record documentation protocols are complied with. Ensures that persons' assessment information is entered into the medical records on a timely basis and in accordance with established organizational and departmental procedures.
  • Ability to effectively organize time, responsibilities, and accomplish task by established deadlines and timeframes.
  • Works effectively and collaboratively in a multi-disciplinary team to insure excellent patient experience.
  • Participates regularly in departmental and staff meetings.
  • Demonstrate understanding of sound ethical practice in conjunction with the established and defined ethics of one's training, discipline, and education/license level.
  • Participates regularly in departmental and staff meetings. Serves on programs or organizational committees as required and as time and workload allow.
  • May provide training to interns and on boarding clinicians as assigned.
  • Responsible to uphold the healing ministry of Christ through relationships that honor integrity, stewardship, empowerment, professional excellence, and the promotion of diversity, equity, and inclusion.
  • Must maintain annual training and demonstrate competency in the required programmatic training.
  • Performs other duties or functions as assigned
Required/Desired Qualifications:

Education, Training, and Experience:
  • Bachelor's degree in social work, psychology or related field required.
  • A minimum of 1 year related work experience related.
  • Experience working with Eating Disorders preferred.

  • Verbal and written communication skills which enable the worker to convey clear, thorough, necessary and timely information to individuals, family, colleagues, supervisory and management personnel.
  • Analytical skills necessary in order to synthesize a variety of clinical and social data to determine appropriate diagnosis, and level of care recommendation.
