Description

The Perinatal Community Health Worker collaborates closely with the Perinatal Team to address the social determinants of health that impact our pregnant patients and their families. This position is integrated into the perinatal care team.

Essential Functions

  • Case management of clients related to social determinants of health in perinatal period
  • Collaborate with perinatal team and partner with clients to manage and implement Care Plans
  • Identify barriers and design interventions to facilitate access to healthcare and wellness in clients' lives.
  • Maintain accountability to tracking data related to social determinants of health.
  • Coordinate monthly health fairs at MCC for perinatal families that provide access to community partners such as MNSure, WIC, Public Health Nurse, Food shelves and other community resources.
  • Do home visits for clients with significant barriers to care. This includes patients who have not been able to attend traditional clinical visits despite providing other resources.
  • Assist in the facilitation of group prenatal care known as Circle of Mothers.
  • Performs miscellaneous job-related duties as assigned.

Supervisory Responsibilities

This position does not have any supervisory responsibilities.

Work Environment

This job operates in a healthcare setting. This role requires regular walking to various locations around the clinic and the community. This role also routinely comes into contact with patients who may have contagious illnesses.

Physical Demands
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.
Travel Requirements

Must have the ability to travel between MCC sites and patients' home throughout the St. Paul/Minneapolis metro area.

Qualifications

Required Education and Experience
  • High School Diploma, GED or higher education.
  • Current CPR certification.
  • A valid Driver's License and reliable transportation
Preferred Education and Experience
  • Community Health Worker Certification preferred. Commensurate work experience or Perinatal Doula training or Medical Assistant experience will also be considered.
  • Bilingual Spanish/English preferred but not required
Additional Eligibility Requirements
  • Demonstrated success in working effectively with target population(s).
  • Change Agile; ability to operate in the gray and flex to new developments or situations.
  • Experience working in a multi-site environment is highly desired.
