Medical Social Worker BS

Job Summary:
Works with Social Service Manager in coordination with Physicians, Care Management Staff, and Nursing Service in providing social services to hospital patients and their families. The position requires knowledge of the age and developmentally specific needs of the child through geriatric population.
Essential Job Duties:
- Discharge Planning - Performs assessments and coordinates planning to assure patient's health needs are met on discharge. Involves patients and families in all planning. Participates in interdisciplinary rounds to develop the most appropriate care plan.
- Counseling - Counsels and provides support to patients and families as appropriate. Refers to support groups.
- Assessment and Referral - Coordinates referrals to a wide variety of community agencies and facilities to meet post hospital needs of patients.
- Emergency Service - Provides social services to Emergency Service patients as needed.
- Documentation - Documents on all cases in a timely and accurate fashion according to Social Service and Hospital policy.
- Conducts child and adult abuse investigations and makes referrals to appropriate agencies for follow up.
- Adoptions - Coordinates adoption cases as needed, providing counseling and coordination of details.
- Performance improvement - Participates in P.I. projects as requested by Social Service Manger.
- Financial - Counsels patients and families as needed regarding disability, Medicare, Medicare And any financial resources that might be available.
- Community Education and Special Projects. - Works with Social Service Manager in Development and implementation of Community Education Activities and special projects.
- Completes other duties or special projects as assigned.
Qualifications
Job Specifications:
- Education: Bachelor's Degree in Social Work or related field. Licensure LSW
- Experience: Two years of hospital social work experience preferred.
