Description Provides street and site outreach to homeless adults with serious mental illness. Assists consumers in accessing needed psychiatric treatment, housing, medical, social, educational, vocational, and other supports necessary to meeting basic needs. Completes clinical documentation in accordance with agency guidelines. CSB Staff roles may include assisting other units at times to assist them with coverage needs, specialized assistance, or to cross-train. Staff hired into direct service positions (direct service, monitoring direct service, supervising direct service) can work in units other than their primary role to provide administrative or direct service activities within their skill set, education, and training. They may also serve in "voluntary" activities outside of the CSB's core services working with the public for community outreach events, service fairs, homeless outreach activities, homeless day shelters, neighborhood outreach events, trainings, etc. in a direct service, clinical, or administrative role. Reports to Programs Manager, Housing and Homeless Services Requires management of a number of competing work priorities in a time-restricted environment. Works with a challenging population, often from diverse backgrounds experiencing homelessness, mental illness and/or substance abuse issues. Provides services in the community. Work hours are full-time, with benefits. Maybe required to work a flexible schedule. EXAMPLES OF DUTIES: Conducts street outreach to homeless individuals in the city of Norfolk. Attempts to develop relationships with persons experiencing homelessness. Completes a face-toface assessment to determine functional limitations, eligibility for service, progress, and need areas. Face-to-face contact occurs through street contact, site visits, and office visits, or in another community setting. Determines eligibility for PATH enrollment and enrolls as appropriate. Develops a Plan of Action based on assessment information in collaboration with the consumer and, if appropriate, family or significant others. Reviews and updates plan as needed. Links to services, resources, and supports specified in the Plan. Examples include linking to psychiatric treatment, housing, entitlements, medical care, food, skill training, etc. Assists the individual directly, which may include transportation, for the purpose of obtaining neededresources. Assistance also includes advocating for the consumer in order to procure needed resources or services, while empowering the individual to be as independent as possible. Coordinates services and service planning with other providers within NCSB and with other agencies, and private providers, such as psychiatric hospitals, counselors, vocational programs, doctors, etc., and obtains clinical documentation from other providers to ensure continuity of care. Makes contact with family or significant others to promote implementation of ISP and community stability. Monitors progress, service delivery, physical and mental stability, and consumer satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated. Provides practical guidance and education to guide the individual in problem solving strategies and to develop a supportive therapeutic relationship. This may include encouraging compliance with treatment, discussing relapse prevention strategies, stress management, etc. Works to resolve critical or situations. Will link to emergency services as needed. Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing by completing documentation and submitting filing within 72 hours of providing the service. Completes an accurate record of direct and consumer related service provided on a daily basis. Complies with regulations for HIPAA, mandated reporting of abuse and neglect, consumer confidentiality, and other consumer rights. Maintains clinical data, in accordance with agency requirements, for consumers on assigned caseload. Attends agency staff meetings, case review meetings, supervisory meetings, and training as required. Performs other related duties as assigned. PERFORMANCE RESPONSIBILITIES: Although each position has its own unique duties and responsibilities, the following areas apply to every employee of the Norfolk CSB. Experience in a human services field related to the area of assignment, to include work in case management. Some assignments also require specific experience working with various types of client populations. This position requires at least one year of experience working with a co-occurring population. CERTIFICATE/LICENSE: Possession of a valid driver's license and an acceptable driving record as reported by the Department of Motor Vehicles are required for persons using personal or agency vehicles in the performance of their duties. Knowledge of public health practices; knowledge of HIV etiology and prevention methodologies. ? Effective oral and written communication and interpersonal skills; highly effective organizational skills. ? Skills to make good decisions, motivate and gain the confidence and cooperation of patients and health professionals. ? Ability to maintain confidentiality and comply with information and security policies. ? Ability to recognize, analyze, and solve problems. ? Ability to work independently at various hours in a wide variety of settings including clinical and field. Ability to work as part of a team; respond to inquiries in a professional and timely manner; interpret and apply policies and procedures; and prepare reports and correspondence. ? High school (HS) diploma or General Education Development (GED) and one year of experience working with persons living with HIV and/or health care training for example certified medical assistance or medical clerk. ? All Non-Medical Case Manager/Eligibility/Intake Specialists complete a minimum training regimen within one year of hire date that includes: (a) AIDS Drug Assistance Program (ADAP) requirements and application, (b) Medicaid, Medicare, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), (c) VDH’s HIV Case Management standards (d) cultural competency, and (e) RW eligibility. If newly-hired Non-Medical Case Manager/Eligibility/Intake Specialists have previously obtained all of the required training, they do not need to repeat it. Documentation of completion of required trainings must be kept in the Non-Medical Case Manager/Eligibility/Intake Specialist's personnel file. ? Five hours of continuing education in HIV/AIDS is required annually. Ongoing training on changes to benefit program and their eligibility, such as Medicare, Medicaid, SSI, SSDI, RW etc. is also required annually. Documentation of completion of required trainings must be kept in the Non-Medical Case Manager/Eligibility/Intake Specialist's personnel file. ? A valid State Driver’s License is required to operate a state vehicle or if operating a personal vehicle while conducting business on behalf of the agency. Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check, proof of education (diploma or transcripts), professional licensure verification, and E-Verify.
