As a key member of the Sentara Healthcare team, the Inpatient Case Manager RN plays a vital role in ensuring seamless patient transitions across the healthcare continuum. This dynamic professional is responsible for providing comprehensive care management services, promoting effective utilization of health services, and collaborating with the healthcare team to manage care and transitions.
Key Responsibilities:- Develop and implement comprehensive care plans based on patient assessments and evaluations, incorporating physical, mental, and psychosocial needs.
- Collaborate with the healthcare team to ensure clinical appropriateness and length of stay (LOS) for patients.
- Directly support in-hospital patients identified as needing comprehensive case management services.
- Monitor and promote effective utilization of health services, ensuring optimal patient outcomes.
- 18 months of nursing experience.
- Registered Nurse (RN) licensure in North Carolina or Virginia.
- Basic Life Support (BLS) certification within 90 days of hire.
- BSN preferred; all RNs without a BSN will be required to sign a BSN Agreement committing to obtaining their BSN within 5 years of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification required within one year of eligibility.
- 1 year of acute care case management experience preferred.
