RN Care Coordinator

The RN Care Coordinator is responsible for organizing and coordinating care to the participants in accordance with the approved Plan of Care under the direct supervision of the Director of Clinical Services and/or Home Care Manager. This position will maintain an open line of communication with the PACE Clinic, PACE Day Center, PACE IDT, and Enrollment to ensure all changes and/or concerns are communicated to the proper clinical team members. The RN Care Coordinator will ensure continuity of care for each participant as they transition into the PACE program to/from outside providers.

What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors’ health so they can live safely in their homes. HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families. This innovative program balances medical, emotional, physical, and social needs of seniors.

What You Bring:
  • RN with current or ability to obtain Colorado licensure.
  • Graduated from an accredited school of nursing; bachelor’s degree preferred.
  • Minimum of 1 year of experience working (paid or volunteer) with the elderly required.
  • Ability to set and maintain professional boundaries with participants and colleagues.
  • Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
  • Ability to participant in on-call rotation
  • Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
  • Ability to work collaboratively and communicate effectively

  • Competitive pay structure starting at $38.50-$40.50 per hour, depending on experience and education
