The MDS Coordinator is responsible for the coordination of the care and accurate completion of the MDS, RAP’s and Care Plans for all assessments; significant change and other resident care services reports. MAJOR WORK ACTIVITIES: Accurately complete comprehensive MDS assessments and significant changes for each resident on the assigned units following the RAI schedule to ensure timeliness. Correct all problems with the Exception Report prior to transmission. Develop a plan of care for each resident in conjunction with the resident, family, and care plan team members based on needs identified through the MDS assessment process and documented in the RAPs. Complete all MDS data and significant change needed to all residents. Complete all clinical chart documentation according to best practice at time service rendered, prior to end of current shift, and/or by “late entry” with date, time, and legible signature or initials where appropriate. May greet the patient and family on arrival and may assist as needed to facilitate a complete admission to facility. Assist Business office by collecting/submitting necessary data to facilitate billing. Contributes to new admission care plans, interview potential residents and residents’ representatives to collect accurate initial data on admissions inquiries. Coordinates with other disciplines to ensure that all parts of the assessments are done in a timely manner. Coordinates all Care Plan meetings for the residents whose data collection he/she was responsible. Enters information on newly admitted residents into the computer system. Produces computer generated reports as needed or requested. Coordinates with finance office to clarify and verify RUG’s. Attends all appropriate inservices as required. Travel to screen possible admissions to facility if needed. Support management/administration decisions through periods of change or stress. Performs specific work duties and responsibilities as assigned by the Administrator. KNOWLEDGE, SKILLS AND ABILITIES: Work is performed indoors with controlled temperatures Exposure to some noise and stress Ability to communicate with people. Physical exertion required no more than 60% of the time. Intermittent lifting and carrying of objects weighing between 30-40 pounds and pushing of objects weighing at least 100 pounds. Physical exertion also is described as standing, stooping, kneeling and bending. Moderate exposure to infectious diseases. Sufficiently mobile to push wheelchairs, carts and equipment. Communicate via telephone and other automation vehicles. Visualize equipment and medical record forms. Ability to monitor call bells and alarms. Must be computer literate and working knowledge of nursing equipment. Ability to evaluate and appropriately respond to verbal and nonverbal communication from residents diverse stages of development, i.e. adults and geriatrics. Physical job demands may be subject to possible modifications to reasonably accommodate individuals with disabilities. MINIMUM QUALIFICATIONS: Education: Requires at least a certificate from an approved school of nursing. Licensure/ Certification: Current licensure as Registered Nurse with the North Carolina State Board of Nursing. Experience: At least have a current Registered Nurse Licensure and one year of previous work experience with MDS is desirable, Resident Care Plans in a long-term care setting.
