Job Summary
The Licensed Practical Nurse (LPN) is responsible for providing nursing care within the scope of practice for a LPN.
Primary Responsibilities
The following duties are primarily performed and which are essential for this position. Employees are expected to be able to perform each of these job duties satisfactorily and successfully to be qualified for the position. Other duties may be required and assigned.
- Administers and documents medications as ordered and under guidelines of LPN licensure.
- Counts narcotics with oncoming shifts.
- Provides routine patient care and treatments including wound care, blood pressure checks, catheterizations, tube feedings, etc. following guidelines and other regulation.
- Takes physician orders over the phone.
- Completes documentation and other paperwork including patient charts, assessments, orders, etc.
- Gathers data for admissions and ongoing care of residents.
- Provides assistance with transferring and transporting patients.
- Provides assistance in transferring and transporting patients.
- Follows universal precautions.
- Responds to resident lights and calls (e.g. wander alarms and motion alarms).
- Recognizes and reports sign and/or symptoms of significant resident health changes, resident abuse or misappropriations and reports changes to the RN.
- Acts as a team leader to Certified Nursing Assistants.
- Attends staff meetings, in-services or other meetings.
- Performs other duties of a comparable level/type, as required.
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
- LPN Diploma from an accredited school of nursing.
- Requires a minimum of one (1) year previous nurse or health related experience.
- Current licensure in the State of WI as a Licensed Practical Nurse
- CPR Certification
In order to perform the duties of the position (listed above) the following knowledge, skills, and abilities are essential for the Licensed Practical Nurse to possess.
- Knowledge of the fundamentals of nursing care practices, techniques, and procedures in providing nursing care and treatments for geriatric, disabled, or impaired residents.
- Knowledge of relevant laws, rules and regulations pertaining to the delivery of nursing services to residents.
- Knowledge and fundamentals of disease processes, issues and problems associated with aging and long-term care needs of residents.
- Knowledge of the use of medical equipment and sterilization procedures.
- Knowledge of medications, drug interactions, medical terminology, and nursing care protocols.
- Knowledge of facility policies and procedures concerning administrative processes and procedures.
- Skill in operating a variety of nursing equipment including oxygen concentrator, IV equipment, suction machines, medical carts, blood pressure cuffs, stethoscope, blood testing equipment, thermometers, lab tools, treatment supplies, feeding tubes, feeding pumps, and tracheotomy cares.
- Skill in applying judgment, discretion and care decisions in addressing resident care and issues.
- Skill in assessing, monitoring, documenting and providing nursing care treatments and medications to residents, per orders (tube feedings, dressings, wound care, lab procedures, etc.) and accordance with LPN licensure requirements.
- Ability to collaborate with other staff, family members and health professional in coordinating resident care services.
- Ability to providing guidance, direction and training to certified nursing assistants.
- Ability to properly lift and use body mechanics.
- Ability to establish and maintain effective and professional working relationships.
- Ability to follow Dunn County policies and procedures.
- Ability to manage time effectively and complete work within established deadlines.
- Ability to work the allocated hours of the position.

In order to perform the duties of the position (listed above) the following mathematical skills are essential for the Licensed Practical Nurse to possess.
- Ability to make arithmetic computations using whole numbers, fractions and decimals.
- Ability to compute rates, ratios and percentages.
- Ability to understand and apply governmental accounting practices in maintenance of financial records.

PHYSICAL REQUIREMENTS
- Physical effort will typically require prolonged and frequent exertion of objects weighing up to 10 pounds, regular, but not continuous, exertion of objects weighing up to 25 pounds, and/or infrequent exertion of objects weighing up to 50 pounds.
- Work requires walking, reaching with hands and arms, speaking and hearing; work frequently requires standing, stooping, kneeling, crouching and crawling; work occasionally requires using hands to handle or feel, climbing and balancing.
- Work has standard vision requirements.
- Vocal communication is required for expressing or exchanging ideas by means of the spoken word.
- Hearing is required to perceive information at normal spoken word levels.
- Duties associated with the job involve some exposure to and environmental conditions such as strong orders, disease/infections/pathogens, and exposure to body fluids, blood, bites and scratches from residents.
- Risks and associated hazards can be minimized but not eliminated through safety training, procedures, equipment and general risk management procedures.

Workers' Compensation
Social Security: The County matches employees' social security compensation.
Vacation: Accrual is based on years of service with the County. For non-management positions, starting at 3.4 hours accruing each pay period (11 days per year), with the maximum accrual rate of 29 days per year. For management positions, starting at 3.08 hours accruing each pay period (10 days per year), with the maximum accrual rate of 29 days per year.
Sick: Accrual is fixed at 3.7 hours accruing each pay period (12 days per year).
Holidays: Ten paid holidays: New Year's Day, Martin Luther King Jr. Day, a Spring Holiday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the Day after Thanksgiving, Christmas Eve Day, and Christmas Day.
