Job Summary:
We are seeking an experienced Hospice Admissions Registered Nurse to join our team at Liberty Hospice. As a key member of our care team, you will be responsible for evaluating and admitting patients according to Local Coverage Determinations (LCD) Guidelines.
Key Responsibilities:
- Evaluate patients for hospice eligibility and develop individualized plans of care.
- Coordinate patient care services, including referrals to other healthcare providers as needed.
- Document comprehensive assessments and patient information in a timely and accurate manner.
- Work collaboratively with the interdisciplinary team to ensure high-quality patient care.
- Participate in on-call rotation and attend monthly staff meetings to stay informed about team initiatives and best practices.
Requirements:
- Current RN licensure within the state of practice with at least one year of clinical nursing experience.
- Successful completion of nursing education from an approved school of nursing.
- Ability to provide effective healthcare services to patients in their home setting, including thorough and accurate documentation of patient visits.
- Strong communication, organization, and time management skills, with the ability to work independently and as part of a team.
- Proficiency in using a personal computer and various software applications, including Windows, Word, and Excel.
- CPR certification and a valid driver's license.
- Commitment to ongoing education and professional development, with a minimum of 12 hours of continuing education annually.
