We are currently hiring a Care Coordinator - Albany, to work in our Care Coordination Services Agency. We are looking for someone who enjoys working in a fast paced and team environment. This position is for someone who is a problem solver and enjoys helping those in need. As the Care Coordinator, you would be responsible for:
- Assess and develop care plans to assist clients in addressing gaps in care and social determinants of health
- Coordinate the care of those with two or more chronic illnesses
- Refer and link clients to needed services and resources
- Work with high-risk populations including clients with, a mental health diagnosis, homelessness HIV/AIDS, Sickle Cell Anemia and/or Substance Use Disorder
- Ability to work in a fast-paced environment
- Complete proper documentation
Care Coordinator - Albany Maximum Salary: $21.02 hourly
Care Coordinator - Albany Schedule: Monday-Friday, 8:30am-4:30pm
Qualifications:
- Bachelor's Degree (preferred but not required) in Health, Education, or Human Services
- 1-3 years of case management experience
- Experience working with diverse populations
