About Us: Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network collaborates to create opportunities for older Ohioans to receive needed home and community services and support; and to age successfully in their own homes and communities.

Position: Non-Exempt; Full-time Salary Range: $24.04 per hour
Job Overview: As a Care Coordinator in the Business Operations Department, you will perform phone assessments to address Social Determinants of Health (SDoH) and link members to community resources and insurance benefits. This role mainly involves working in an outbound call center environment.

- Conduct comprehensive phone assessments with members to identify and address Social Determinants of Health (SDoH) factors affecting their well-being.
- Develop a deep understanding of members' social, economic, and environmental circumstances through effective questioning and active listening.
- Collaborate with interdisciplinary teams to create individualized care plans that address identified SDoH barriers and promote holistic health outcomes.
- Facilitate referrals and coordinate access to community resources such as housing assistance, transportation services, food assistance programs, and mental health support.
- Educate members on available healthcare insurance benefits and assist with enrollment or navigation of insurance coverage options.
- Maintain accurate documentation of assessments, interventions, and outcomes in accordance with regulatory requirements and organizational standards.
- Provide ongoing support and follow-up to ensure members' needs are met and address any barriers to accessing recommended resources or services.
- Stay informed about changes in social services, community resources, and healthcare policies to provide up-to-date information and support to members.
- Capable of effectively managing and efficiently handling a substantial volume of both outbound calls initiated by the representative and inbound calls received from members.
- Collaborate with internal stakeholders and external partners to advocate for systemic changes that address social determinants of health and improve health equity within the community.
- High school diploma or equivalent educational attainment. Preference to associate degrees.
- Previous experience in fields related to social services, human services, community resources, welfare programs, and healthcare.
- Proficiency in conducting verbal and written interviews and assessments with excellence.
- Strong ability to communicate effectively and collaboratively with others, fostering positive working relationships.
- Demonstrated capability to comprehend written materials pertinent to essential job functions.
- Aptitude to understand, recall, and apply information relevant to essential job functions.
- Skill in analyzing, evaluating, and implementing logical courses of action based on available information.
- Basic proficiency in computer skills, including familiarity with Microsoft Office applications.
- Ability to manage high call volumes efficiently using a digital phone system.

Physical Requirements:
May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. Finger dexterity to operate office equipment required. Ability to communicate face-to-face, phone, email, and other communications. Ability to push/pull/lift/25 pounds. Requires some exposure to warm and cold temperatures.
