Care Coordinator

The Cuyahoga County Juvenile Court is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will work with diversion-level youth who display behavioral health needs and/or family turmoil. Your primary responsibility will be to assess and evaluate the needs of these youth while empowering parents to address behavioral health needs, redirecting youth away from the juvenile justice system, and in lieu of formal court processing.
Key Responsibilities- Coordinate and provide services for diverted youth, focusing on assessing their needs and empowering parents to address them.
- Develop strength-based and family-focused goals for alternative case planning, maintaining case management documents for active youth.
- Refer youth and/or their families to services and/or programs sponsored by the Court, community agencies, and/or other approved providers.
- Conduct meetings with families in crisis, establishing safety and community plans, and referring them to appropriate community-based service providers when necessary.
- Assist with completing the Child And Adolescent Needs and Strengths (CANS) Assessment and maintain certification and yearly training requirements.
- Serve as a link between treatment agencies and families, Court, and other entities to ensure all parties are equipped with information for resources.
- Participate in mediation meetings at identified schools to act as a support and collaborator in developing youth educational success planning.
- Work closely with Court personnel and community partners in a team environment.
- Assist with training for police and community partners.
- Bachelor's Degree in Social Science or related field.
- Three (3) years' experience with adolescents with co-occurring disorders, five years preferred.
- Valid Ohio driver's license and maintain valid automobile insurance, with a clean driving record and ready access to a safe functioning automobile.
