Job Summary:

The Therapy Case Manager - Patient Care Coordinator will be responsible for managing all aspects of accepting and managing therapy referrals, scheduling field therapists, and coordinating patient care. This is a temporary position with potential to extend into a permanent one.

Key Responsibilities:

  • Schedule field therapists to patients in accordance with their plan of care.
  • Assist with new employee orientation and coordinate all therapy for patients.
  • Review and understand the admission process for each patient's plan of care.
  • Coordinate plan of care with therapy providers and review therapy evaluations and visit notes.
  • Maintain a good working relationship with contracted companies and assist with additional scheduling responsibilities as needed.

Requirements:

  • High school diploma required; 1 year of Home Health Care experience preferred.
  • Ability to read, analyze, and interpret common patient information, medical documentation, and insurance information.
  • Ability to respond to common inquiries or complaints from clients and therapy providers.
  • Ability to effectively present information to management and co-workers.
  • Proficient in personal computer skills, including electronic mail, record keeping, routine database activity, word processing, and spreadsheet software.

Additional Qualifications:

  • Strong organizational skills and understanding of information storage and retrieval required.
