The Patient Assessment Coordinator will facilitate admissions by coordinating transmission of clinical and benefit information from referral sources back to the SNF admissions teams, administrations, and third party payers. The Payment Assessment Coordinator will preferably have an RN or LPN license, a degree in Social Work, or other medical credentials. Estimated Salary Range: $60k-70k/annually Essential responsibilities include: Performing clinical assessments Resource to the Liaisons ThePatient Assessment Coordinator achieves these objectives by providing excellent, responsive customer service and accurate, timely, and comprehensive clinical assessments and by proactively working with all departments in the facility. Qualified candidates for this position will have an RN or LPN license in the State of Ohio, a Social Work degree, and/or other medical credentials. The ideal candidate will preferably have three years' treatment experience in a Long Term Care setting. Previous clinical screening experience preferred, along with recent healthcare experience in an Long Term Care facility. Candidates will also preferably have knowledge of the local medical community and an outgoing, energetic personality . Job Requirements: Preferred: Current, valid and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, OR Social Work Degree, OR other medical credential. Current, valid and active driver’s license required. Three (3) years treatment experience in a Long Term Care setting preferred. Previous clinical screening experience preferred. Recent healthcare experience in Long Term Care preferred. Knowledge of local medical community preferred. Ability to collect, organize, evaluate and communicate professionally and assertively, both verbally and in writing, pertinent clinical information with medical and administrative staffs, patients and families. Strong customer service skills and commitment to service excellence. Outgoing, energetic personality Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills. Ability to make independent decisions when circumstances warrant. Working knowledge of computer and software applications used in job functions. The Patient Assessment Coordinator is a full time, salaried position.
