The role also coordinates program activities, analyzes data, and organizes/coordinates CHWs caseloads. Role will support data collection and support overall program evaluation. Our organization offers an innovative, evidence-based health care program that improves health outcomes for low-income, high-risk patients using Community Health Workers (CHWs). In this model, CHWs provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals.Minimum Requirements1. High School Diploma required.2. Minimum of 3 year experience with data collection, management, and/or analysis.3. Minimum of 2 year experience extracting information and exporting data to other applications.4.
