We are seeking a highly skilled and compassionate Senior Community Health Worker to join our team at Providence. As a key member of our community health team, you will play a vital role in helping community residents and Providence Medical Foundation patients and their families navigate and access community services, other resources, and adopt healthy behaviors.
Key Responsibilities- Health Education: Assist with recruitment and administrative tasks for health education courses, such as Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. Deliver these modules independently and identify new locations where the programs can be delivered.
- Outreach and Enrollment: Provide outreach in community and PMF settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). Provide one-on-one health insurance and/or CalFresh enrollment application assistance. Troubleshoot complex cases independently and provide small group presentations on how to use insurance and basic health care rights.
- Navigation/Linkage to Resources: Manage complex patient subpopulations, including people with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse.
- Education: Meet certification, license or registration requirement.
- Experience: 2 years of experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelor's degree in lieu of experience.
