Key Responsibilities:

The following responsibilities are typical for this role; additional tasks may be assigned as necessary:

  • Supervise Assisted Living, Memory Care, and Specialty Care staff, including training, task assignment, and performance reviews.
  • Monitor the health and wellness of residents, ensuring proper care including medication management and dietary needs.
  • Collaborate with the administrator on staff scheduling and participate in evaluation meetings.
  • Engage with residents, families, and healthcare professionals to provide comprehensive care.
  • Respond to emergency calls as part of an on-call rotation.
  • Identify safety hazards within resident apartments and provide necessary safety instructions.
  • Conduct health assessments, maintain accurate medical records, and manage outpatient logs.
  • Assist visitors and manage phone inquiries at the facility.
  • Facilitate training sessions for care providers.
  • Coordinate transportation for residents to medical appointments.
  • Attend training workshops and meetings to enhance professional skills.
  • Adhere to safety protocols and report any issues to the administrator and cooperating RN.
  • Ensure residents' rights are upheld and provide courteous service to all.
  • Collaborate with the administrator on admissions and ensure compliance with service plans.
  • Monitor resident charts and clinical notes for ongoing assessments.
  • Communicate changes in resident needs to physicians and other relevant personnel.
  • Be available for on-call emergencies, carrying a cell phone as needed.
  • Coordinate educational opportunities for staff, ensuring compliance with state and federal regulations.
Qualifications:

To be considered for this position, candidates must possess:

  • A valid Registered Nurse (RN) license.
  • A Bachelor's degree in Nursing is preferred, although an Associate's degree or diploma in Nursing is acceptable.
  • A minimum of five years of nursing experience in a similar environment.
  • Strong knowledge of medical procedures and excellent clinical assessment skills.
  • Proficiency in electronic health record (EHR) systems.
  • Effective communication skills with patients, families, and healthcare teams.
  • A compassionate approach to patient care.

Work Environment: This role requires the ability to work effectively in both residents' apartments and assisted living facilities, with potential exposure to various health-related environments.
