- Oversees staff in Assisted Living, Memory Care, and Specialty Care, including training, task assignment, and performance evaluation.
- Evaluates and monitors the health and care requirements of residents, including medication management and dietary needs.
- Collaborates with administration to coordinate staff schedules.
- Participates in assessments of both staff and residents.
- Engages with residents, their families, and healthcare professionals.
- Responds to emergency calls as part of an on-call rotation.
- Identifies safety issues within residents' living environments and provides necessary safety guidance.
- Conducts health evaluations, maintains medication records, and processes physician orders.
- Assists visitors and responds to phone inquiries at the facility.
- Facilitates training sessions for all care staff.
- Arranges transportation for residents to medical appointments.
- Attends training workshops and meetings as feasible.
- Ensures adherence to safety protocols and reports to the administrator and collaborating RN.
- Delivers courteous and effective service to residents, guests, and colleagues.
- Advocates for the rights of all residents.
- Performs other essential duties as required.
- Collaborates with the administrator on the AWAIR program and HIPAA compliance.
- Assists with admissions and ensures compliance with service plans.
- Fills in for absent staff as needed, in conjunction with the administrator.
- Reviews residents' charts and clinical notes.
- Guides residents through the admission process and transition phases.
- Communicates changes in residents' needs to physicians, administrators, and case managers, tracking referrals and follow-ups.
- Available for on-call emergencies, carrying a cell phone as needed.
- Coordinates staff education, ensuring compliance with annual training requirements.
- Maintains facility compliance with state, federal, and local regulations, and is prepared for licensure surveys.
- Shares staff scheduling responsibilities with the administration.
- Current Registered Nurse (RN) license in the relevant state.
- Bachelor's degree in Nursing preferred; Associate's degree or diploma in Nursing accepted.
- A minimum of five years of nursing experience in a comparable environment.
- Strong understanding of medical procedures, treatments, and medications.
- Excellent clinical assessment skills with the ability to make informed decisions in emergencies.
- Proficient in electronic health record (EHR) systems.
- Effective communication skills with patients, families, and healthcare teams.
- Compassionate approach and commitment to high-quality patient care.
Ability to adapt to various settings, including residents' apartments and healthcare facilities, while maintaining safety and health standards.
