Position Overview:
The Care Coordinator is responsible for providing wraparound case management services to students enrolled in Hennepin County's Plus program who are currently attending Menlo Park Academy. The goal of the position is to help students achieve academic success and reach their personal goals. The Care Coordinator will consult with staff from Menlo Park Academy and Hennepin County, as well as parents to ensure student success in the program. They will support student engagement, help address mental and behavioral health concerns, and coordinate referrals to other supportive services in the community, transporting youth as needed. The Care Coordinator will also help students transition to mainstream education settings as appropriate.
Key Responsibilities:
- Case Management Services - 65%
- Provides guidance, support, and case coordination to students referred to the ALC Plus Program, as well as their parents/guardians and other stakeholders.
- Promotes positive social skills and appropriate pro-social behavior with referred students.
- Monitors student academic performance, behavior and attendance and support interventions in conjunction with Menlo and ALC Plus team members.
- Assists and supports students who are experiencing emotional, behavioral, or health-related issues and/or students who have difficulty attending school on a consistent basis.
- Supports, tracks and assists with school-related challenges for students referred to the Plus Program.
- Participates in recreational and educational afterschool activities for students.
- Develops professional caring relationships with program participants and families.
- Collaborate with Internal and External Partners - 25%
- Consults and collaborates effectively with parents/guardians, educational staff, administrators and other educational/community resources regarding students with identified concerns and issues.
- Introduces and refers students to resources that may assist them with personal and educational issues.
- Travels to home and community-based settings to provide services, as needed.
- Transports program participants to educational settings, court appearances and home if necessary.
- Attends staff development opportunities, including mandatory trainings required by Hennepin County.
- Assists with the identification of unmet social/emotional needs of students and make recommendations/referrals for further assessments and services.
- Participates in the integrated/multi-disciplinary intake team to assist in the development of a complete Student Service Plan and other intake paperwork.
- Participates in school staff meetings, case consultations and student conferences as needed.
- Assists families in connecting with community resources.
- Develops and utilizes available area resources and make appropriate referrals.
- Serves as an appropriate professional role model for parenting students, encouraging healthy parenting strategies for their children, and providing referrals to additional parenting education as needed.
- Data Collection - 10%
- Collects and accurately interprets, utilizes and reports student data as required.
- Collects and monitors designated data and presents data in staff meetings. Prepares reports as required.
- Maintains case notes documenting contact with program participant, family and collateral contacts.
- Ensures participants confidentiality by following HIPPA/FERPA data privacy and security regulations.

Education and Experience
- Bachelor's Degree in Human Services or related field (required by funder)
- Prior experience with youth age 16-21 (preferred)
- Prior experience with the education, juvenile justice, and/or child welfare systems (preferred)
- Intermediate level computer / data entry skills
- Demonstrated commitment to the organization's mission
- Excellent problem-solving and analytical abilities
- Demonstrated communication skills with the ability to interact professionally with diverse individuals and organizations
- Demonstrated organizational skills and ability to maintain accurate and timely records
- Self-motivated with a dedication to continuous improvement
- Valid MN Driver's License and proof of auto insurance. Dependable transportation is required.
Working Conditions
This position operates in a secure, accessible, and smoke free office setting, with occasional travel to other community-based sites across the Twin Cities. Work hours are year-round, typically Monday through Friday, between the hours of 8:00 AM to 5:00 PM with occasional evenings and weekends as required. When school is in session, this position is expected to work in-person to best serve the needs of students, and the opportunity to work remotely during school release days and summer break. Must have ability to sit or stand for long periods of time and utilize various office equipment (e.g., computer, printer, phone, and calculator) on a continual basis. Must have the ability to lift up to 35lbs as needed.
Compensation:
This is an hourly nonexempt position with a competitive benefits package. The hourly rate of pay is between $25.00-27.00 depending on qualifications. This full-time position is 40 hours per week with occasional evenings and weekends as needed, Monday-Friday, hours are based on program needs.
