Charting at least quarterly and as necessary according to the MDS schedule. Charting on restraints; charting on behaviors and interventions and other changes in status as necessary.Assure that social service progress notes are informative and descriptive of the services provided and of the resident's response to the service.Interview resident and family members to obtain social history for initial assessment and plan for care. Involve the resident and family in planning objectives and goals for the resident.Knowing which residents vote and obtaining an absentee ballot for them; If not registered and resident wants to vote, contact clerk's office and get the resident registered and obtain absentee ballot.Participate in community involvement related to the interests of the facility and the services and needs of the resident and family. Conduct discharge planning, contacting the physician for a discharge order; faxing MARS to the physician so that current prescriptions can be called in to the residents choice of pharmacy; scheduling home health to set up care and the equipment/supplies which the resident may need for home care.Awareness of appropriate resources and agencies which may be able to assist residents.Knowledge of local and state programs which may be able to assist residents.Ability to understand medical treatment objectives in order to explain them to the resident and family.Assisting the resident and their families in dealing with grieving, family issues, and crisis situations resulting from or which may affect the resident's physical or mental health.Scheduling of interdisciplinary care plan meetings for all residents at least quarterly. Mailing of care plan invitation letters to families and notifying the resident of the meeting and inviting them to attend. Keep record which includes care plan meeting minutes and attendance.Conducting in-services with all personnel on, but not limited to; abuse, resident rights.Assist with activities and special events as needed.Arrange psychiatric consults as needed.Assist in developing a written plan of care for each resident that identifies the problem and needs of the resident and the goals to be accomplished for each problem or need identified.Other duties as assigned.Physical Demands and AbilitiesThe physical demands and work environment characteristics described herein are representative of those that must be met by an employee to successfully perform the essential duties of this job.
