Starting Pay: $20.61-23.70

Who We Need
The Intake Specialist develops the initial integrated comprehensive assessment and Integrated Service Plan (ISP) for new service recipients. Completes intake packet, including, but not limited to: consents, member handbooks, notice of privacy practices, releases of information, etc. Assists service recipient in Patient Portal enrollment. Completes initial referrals in accordance with goals from the initial Integrated Service Plan (ISP). Provides all new referral information to Integrated Case Manager and care team for their ongoing care. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation

Minimum Qualifications:
  • Bachelor's degree in behavioral health related field, and (2) two years of full-time work experience in behavioral health.
  • Equivalency of a HS Diploma/GED and (6) six years of full-time behavioral health work experience acceptable.
  • Bilingual (English/Spanish) preferred
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of
  • Must have reliable transportation, valid AZ Driver's License with state minimum insurance coverage and ability to meet and maintain Copa Health's acceptable driving requirements.
  • TB Test clearance required at the time of hire and annually.
Job Summary (Including, but not limited to)
  • Develops the initial comprehensive assessment and Integrated Service Plan (ISP) within required timeframes. Completes all intake paperwork/ packets and facilitates initial referrals needed based on ISP goals.
  • Works collaboratively with the integrated care team to educate and communicate initial treatment needs and goals from Integrated Service Plan (ISP) of new service recipients.
  • Completes the initial Comprehensive Assessment and service delivery plan based on specified health, wellness and behavioral health goals. Collaborates with the new service recipient, family and/or significant others to implement an effective service plan, explaining the available clinical options.
  • Provides screenings and evaluations for new referrals, including counseling, residential and supportive services and community resources. Makes recommendations for initial authorization of service referrals. Completes initial referrals in accordance with goals from the ISP.
  • Completes intake packet, including, but not limited to: consents, member handbooks, notice of privacy practices, releases of information, etc.
  • Provides education of and assists service recipient in Patient Portal enrollment.
  • Orients service recipient to the outpatient site, team, services, etc. and provides education to T19 recipients regarding on-site Primary Care services.
  • Serves as the initial point of contact for new service recipients via telephone or face-to-face interactions with a person, family or other involved party for the purpose of facilitating new enrollment.
  • May be required to assist clinical team with routine, yearly comprehensive assessment and Integrated Service Plan (ISP) completion in order to ensure contact compliance expectations are met.
  • Coordinates and communicates with Clinical Director, Clinical Coordinators, care team, nursing and medical and psychiatric practitioners for individuals exhibiting urgent or emergent medical and/or psychiatric symptoms.
