Position Summary:Kendal at Lexington is seeking a dedicated and compassionate Care Coordinator to join our team on a part-time basis. In this role, you will promote the psychosocial well-being of our Residential Living residents through effective case management and support services. Collaborating closely with the Quality Assurance Manager, you will play a vital role in ensuring continuity of care for our residents.Qualifications:Education: Minimum of a Bachelor's degree in Social Work, Sociology, or a related field.Experience: At least two years of professional experience working with aging populations is required; experience with individuals with dementia is preferred.Essential Functions:1. Support for Residential Living Residents:Monitor and assess resident status to identify changes that may require increased support or services.Provide psychosocial, emotional, and spiritual support through educational sessions, support groups, and outreach efforts.Collaborate with the team to complete required assessments, such as Resident Functional Screens during move-ins and throughout residents' stays.Engage with the Welcome Committee to lead the Kendal 101 orientation series for new residents.Serve as a liaison between the Resident Association and Kendal at Lexington, supporting association initiatives, attending meetings, and assisting with resource updates (e.g., Resident Handbook and CATIE web platform).Conduct regular wellness visits and provide interventions or counseling to high-risk residents, helping them live life to the fullest.2. Transition Support Across the Continuum of Care:Assist new residents with orientation and integration into the Kendal community through visits and ongoing support.Provide assistance to Independent Living (IL) residents through phone calls, emails, and visits as needed.Collaborate with team members to address changes in resident health or care needs and assist with emergencies as appropriate.Support the team in managing discharges and transitions for residents moving between levels of care.Provide counseling to residents and their families, addressing social, emotional, and psychological needs, and help develop personalized care plans in coordination with multidisciplinary teams.Advocate for residents' rights, ensuring they receive high-quality care and maintain an optimal quality of life.3.
