Social Worker

We are seeking a dedicated and compassionate Social Worker to join our healthcare team. The ideal candidate will have a Master's degree in Social Work, Counseling and Guidance, or a related field from an accredited university, along with 3000 hours of post-graduate experience. The Social Worker must be licensed and possess the training, knowledge, and skills necessary to provide evaluation, treatment, and patient care for patients across various age groups, including pediatric, adolescent, adult, and geriatric populations. Key Responsibilities: Conduct comprehensive evaluations of patients to assess their social, emotional, and environmental needs. Develop and implement individualized treatment plans in collaboration with the healthcare team. Provide counseling and support to patients and their families, addressing psychosocial issues and enhancing coping strategies. Facilitate access to community resources and services, such as housing, financial assistance, and support groups. Advocate for patients' rights and needs within the healthcare system and the community. Participate in multidisciplinary team meetings to coordinate patient care and ensure holistic treatment approaches. Maintain accurate and timely documentation of patient interactions, treatment plans, and progress notes. Stay informed about best practices and advancements in the field of social work and integrate this knowledge into patient care. Qualifications: Master's degree in Social Work, Counseling and Guidance, or a related field from an accredited university. Minimum of 3000 hours of post-graduate experience. Current state licensure as a Social Worker. Experience in providing care for pediatric, adolescent, adult, and geriatric patients. Strong assessment and diagnostic skills. Excellent communication and interpersonal skills. Ability to work effectively as part of a multidisciplinary team. High level of empathy, compassion, and professionalism.
