- In this position, the Social Worker will be responsible for Hospice Patient assessment and implementation of plan of care.
- The Social Worker will work provide direct Case Work Services.
- Master's Degree in Social Work required
- WI/IL Social Work License is required
- State of WI letter approval of Psychotherapy Services in Certified Out-Patient Mental Health Clinic
- Evaluation and treatment for pediatric, adolescent, adult and geriatric population
- Valid Driver's License.2 years or more of experience is preferred.
