_We are seeking a dedicated and compassionate Long-Term Care Licensed Practical Nurse (LPN) to join our team. The LPN will play a vital role in delivering high-quality care to residents in our long-term care facility, ensuring their safety, comfort, and well-being. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for working with elderly patients._

*Key Responsibilities:*
* *Resident Care:* Provide direct nursing care to residents, including administering medications, monitoring vital signs, assisting with personal hygiene, and performing basic wound care.
* *Care Coordination:* Assist in the development and implementation of individualized care plans in collaboration with other healthcare team members.
* *Monitoring and Reporting:* Observe and document residents' health conditions, identify any changes or concerns, and report to the supervisor.
* *Medication Administration:* Safely administer prescribed medications and monitor residents for any adverse reactions, maintaining accurate medication records.
* *Team Collaboration:* Work collaboratively with other healthcare professionals to ensure cohesive and comprehensive care for residents.
* *Documentation:* Maintain accurate and thorough documentation of all care provided, including resident observations, care activities, and changes in condition.
* *Resident Advocacy:* Advocate for residents' needs and preferences, ensuring their dignity, privacy, and rights are upheld.
* *Family Communication:* Communicate effectively with residents' families, providing updates on their loved ones' health status and care plans.
* *Safety and Compliance:* Adhere to all facility policies, procedures, and regulatory standards, including infection control and safety protocols.
* *Emergency Response:* Respond to emergencies promptly, providing appropriate care and notifying the supervisor as necessary
* *Education:* Educate residents and their families about health management and care procedures as needed.

*Qualifications:*
* *Education:* Graduate of an accredited LPN/LVN program.
* *Licensure:* Current and valid Licensed Practical Nurse (LPN) license in Wisconsin or compact license.
* *Experience:* Minimum of 1 year of experience in long-term care or a similar healthcare setting preferred.
* *Certifications:* Basic Life Support (BLS) certification required.
* *Skills:* Strong clinical skills, excellent communication and interpersonal skills, ability to work effectively in a team, attention to detail, and a compassionate approach to care.
* *Knowledge:* Understanding of long-term care regulations, infection control procedures, and best practices in geriatric care.

$42.00/Hr. (Blended Rate)
