Amergis is seeking a skilled Long Term Care RN to join our team. As a Long Term Care RN, you will be responsible for providing high-quality patient care in a Long Term Care and/or Assisted Living Facility.
Key Responsibilities- Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare team members and document actual and/or potential nursing diagnoses.
- Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions as well as responses to interventions.
- Perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards.
- Provide a safe, comfortable, and therapeutic patient environment.
- Initiate and evaluate patient and/or family education.
- Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities.
- Monitor safety and effectiveness of the environment and equipment.
- Participate in activities to facilitate good interpersonal communications with the chosen facility.
- Support professional practice by observing the following: uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patients Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies.
- Current Registered Nurse License for the state in which the nurse practices.
- Complies with all relevant professional standards of practice.
- Participation and completion of Amergis' Competency program when applicable.
- Current CPR if applicable.
- TB Questionnaire, PPD or chest x-ray if applicable.
- Current Health certificate (per contract or state regulation).
- Must meet all federal, state, and local requirements.
- Successful completion of new hire training as applicable to job site.
- Understand patient confidentiality and HIPAA requirements.
- Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
- Computer proficiency required.
- Must be at least 18 years of age.
