THE POSITION :

The Interim Housing Case Manager is responsible for assisting participants with assessing and connecting to community resources. The Interim Housing Case Manager is responsible for providing support and advocacy through crisis prevention and intervention, conflict resolution, and intensive case management. The Interim Housing Case Manager must be able to complete thorough needs assessments, maintain/obtain necessary documentation, and collaborate with various community-based organizations to achieve connection to services.

WHAT YOULL DO :

ASSESSMENTS & INDIVIDUALIZED PLANNING

  • Assess participant needs and develop individualized plans to address goals, such as income, medical, mental health, substance use, education, childcare, and social support.
  • Provide ongoing strength-based case management services and coordination with participants.
  • Conduct crisis and risk assessments, and develop safety plans as needed in consultation with supervisors.

RESOURCE REFERRALS

  • Offer referrals, linkages, information, and support to connect participants with needed resources.
  • Collaborate with internal and external providers to ensure effective care coordination.

RECORD KEEPING & DOCUMENT COLLECTION

  • Assist participants in obtaining necessary documentation for housing connections.
  • Maintain accurate records using written forms and HMIS/CHAMP.

CRISIS INTERVENTION

  • Provide crisis intervention and conflict management support when necessary.
  • Follow mandated reporting guidelines, communicating with protective and emergency services as required, and completing follow-up documentation, including incident reports.

OTHER

  • Attend regular staff, agency, and community meetings as directed by supervisors.
  • Stay informed about best practices and interventions related to Domestic Violence, Harm Reduction, Trauma-Informed Care, and the Housing First Model.
  • Additional tasks, projects, and responsibilities as assigned by supervisors.

WHAT YOURE SKILLED AT :

  • Preferred experience in advocacy and case management
  • Strong problem-solving abilities
  • Exceptional time management proficiency
  • Skilled in conflict resolution
  • Excellent customer service aptitude
  • Capable of providing crisis interventions
  • Proficient in Microsoft Office Suite (Word, Excel, Access, PowerPoint), Zoom, and web-based scheduling software
  • Effective written and verbal communication skills
  • Bilingual in Spanish/English is highly desired
  • Demonstrated leadership, teamwork, and independent work capabilities
  • Strong organizational skills with a commitment to task and project completion

OTHER:

  • Must be able to work a flexible schedule at multiple sites including evenings and weekends
  • Maintain and execute confidential information according to HIPAA standards
  • Obtain and maintain CPR/First Aid Certification
  • Ability to pass a Pre-Employment Physical Exam
  • Ability to pass post-offer Tuberculosis (TB) clearances
  • Travel is a regular duty for this position and is required 5-10% of the time (depending on assigned location/department)
  • Use of a personal vehicle to travel between worksites and other locations is required
  • Must have and maintain a valid California Drivers License and auto insurance in good standing

WHAT YOUVE ACCOMPLISHED :

  • At least one (1) year of relevant work experience
  • Experience in dealing with substance abuse, mental health, and crisis intervention desired
