The RN Care Coordinator will coordinate patient care across various settings, develop and implement discharge plans, and collaborate with patients, families, and healthcare team members. This role involves educating patients on disease management and assisting in healthcare navigation. Strong organizational, communication, and computer skills are essential, along with a valid Maine RN license and 3-5 years of relevant experience.

Requirements
- Current RN licensure in Maine.
- AHA BLS certification.
- 3-5 years of experience in acute care nursing, home health, or care management.
- Strong organizational, communication, and computer skills.
- Valid Maine driver's license and reliable vehicle.
- Coordinate patient care across inpatient, outpatient, and community settings.
- Develop and implement discharge/transition plans.
- Collaborate with patients, families, healthcare team members, and community resources.
- Educate patients on medications, disease conditions, and self-management skills.
- Utilize assessment and risk assessment tools to identify healthcare needs.
- Provide POCT services for diabetes management, anticoagulation therapy, and medication dose regulation.
- Participate in diabetes education programs and manage patients on anticoagulation therapies.
- Crosstrain in all areas of care management.
