About The Organization

SaVida Health, a private equity backed healthcare company, provides outpatient opiate and alcohol addiction treatment services. SaVida Health's care model includes medical care, counseling, comprehensive toxicology testing, case management and medical management of psychiatric medications. SaVida is headquartered in Nashville, TN and currently operates in Massachusetts, Maine, Delaware, Vermont, New Hampshire, Tennessee and Virginia and is developing the capability to expand rapidly to meet the needs of patients suffering from opiate and alcohol addiction.

Care Coordinator will be responsible for assessing, planning, implementing, monitoring and evaluating actions required to promote patient’s physical, social, and emotional health in an outpatient treatment setting.

Role And Responsibilities

Completing psychosocial assessments as needed Conduct strengths-based treatment planning per established guidelines Perform care coordination with outside providers Assist with patient health promotion and wellness as requested Ensure all company’s policies and procedures as they pertain to patient’s engagement in treatment are upheld Develop collaborative relationships with members of multidisciplinary teams to support strong patient outcomes. Establish a network of connections and effective relations with community agencies, hospitals, and service providers to effectively link patients with services that meet their needs and strengthen their stability and independence. Provide continuity of care as evidenced by documentation of and communication with collaborative providers and agencies Act as patient advocate to coordinate required services or to resolve emergency problems in crisis situations. Participate in ongoing care collaboration meetings, staff meetings and professional development opportunities Participate in both Individual and Group supervision as scheduled Ensure and maintain the confidentiality and safekeeping of all treatment related documentation. Maintain confidentiality Perform other duties as assigned

Qualifications And Education Requirements

Bachelor’s Degree in counseling, social work or other allied mental health field One-year experience working in the substance abuse treatment field One-year experience working in the mental health counseling field MHRTc license required

Preferred Skills

Ability to maintain professionalism in workplace including professional boundaries and codes of ethics Ability to work independently and as part of a team Ability to plan and prioritize work with limited supervision Working knowledge of the social services, justice, health, community service, and governmental agencies a plus Serve as liaison to participants, monitors, employees, and professional liability insurance providers and provide advocacy as needed Ability to communicate well, have the investigative know-how, and ability to work with individuals to may exhibit strong personalities One-year experience using an Electronic Health Record. Previous use of Athena Electronic Health Record preferred Computer literacy; typing skills, knowledge of EHRs, MS Word, MS Excel preferred Ability to work with individuals and groups from diverse populations; bilingual/bicultural preferred Excellent verbal and written communication skills Ability to present information in a group setting Ability to effectively manage time to meet deadlines Proficiency in clinical documentation (assessment, treatment planning, progress notes) Maintain and support the mission and goals of the organization
