The Manager promotes teamwork, collaborative practice within the team and with community partners, and ensures compliance with regulatory conditions of participation related to patient rights and discharge planning.Essential Duties and ResponsibilitiesMaintains departmental schedule, including daily staffing, holiday rotations, and reporting absences to DPR specialist for tracking.Facilitates daily huddles, attends bed huddles, communicates key patient information with nursing leadership, rounds with staff in all settings, and solves problems.Assists with performance appraisal process, identifies areas needing improvement, coaches staff, seeks common ground to provide conflict resolution, and focuses on positive outcomes.Collaborates with director to update departmental resources, standards of work, and policies.Continuously monitors for compliance with discharge planning, conditions of participation and patient rights, outpatient regulatory compliance related to SDOH, TCM/CCM billing practices and collaborates with department director to implement changes.Serves as liaison between community partners and the organization in regards to discharge planning, health equity and case management practices.Participates in committees such as Stroke/Stemi, readmission team, UM, and other organizational initiatives representing departmental leadership.EducationBachelor's degree in Nursing, Social Work, or related field required. Training in Microsoft EXCEL highly preferred.Work Experience3 years in case management or related field with management/leadership experience.Certification/LicenseMissouri or Compact State RN Licensure if a nurseAmerican Heart Association Basic Life Support (BLS) required.Case Management certification preferredMental/Physical RequirementsConsiderable mental concentration for sustained periods of time with frequent interruptions.
