Complete assigned patient care based on the patients conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report, and record relevant patient information. Participate in peer review and performance improvement activities. Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. Function with an awareness and application of safety procedures. Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Assist with providing training and/or clinical direction as applicable to supporting government HCWs (e.g., hospital corpsmen, students, etc.). Attend and participate in various meetings as directed. Perform timely, accurate, and concise documentation of patient care. Operate and manipulate automated systems such as ADS, MHS Genesis participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications. Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions.Ensure patient care is carried out in accordance with the Standards of Nursing Care and the policies and procedures of the clinic. Check patients into the clinic and assess using clinic guidelines. Inform the nurse or physician of the patients condition and potential problems. Obtain and document patient and pertinent family history. Perform a full range of diagnostic support duties which include taking vital signs, collecting specimens, obtaining, recording, and tracking results of diagnostic tools. Assist the physician in the completion of procedures by preparing, positioning, and monitoring patients, and setting out and handling instruments and equipment. Perform laboratory tests or finger stick such as checking for sugar (glucose) . Record and report results of test. Perform a range of treatment procedures that include sterile dressing changes, applying compresses, monitoring IV fluids, IV catheter placement, administering meds, setting up oxygen and suction, inserting nasogastric tubes, administering medications, giving enemas, setting up and giving treatment that requires auxiliary equipment such as oxygen and suction. Ambulate patients to other areas to include Radiology, various clinics, and physical therapy. Prepare, start, monitor, and discontinue intravenous fluids with accuracy and in accordance with established procedures. Observe, report and document all observed symptoms, reactions, treatments, and changes in the patient condition to the registered nurse or physician. Make careful observations to assess that nursing procedures and treatment do not cause additional distress. Perform routine nursing care activities such as taking blood pressures, temperatures, passing and removal of trays, changing of linen, and otherwise assist in the care of the physical needs of the patient. Operate basic equipment required in delivery of patient care such as pumps, IV pumps, oxygen administration apparatus. Execute physicians orders within the guidelines of standard nursing practice. Ensure accurate medication is administered in correct form and dosage to the proper patient as directed by the physician. Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason. Recognize conditions which require isolation. Ensure universal precautions are used in all patient encounters. Recognize emergency situations and assist with, or institute emergency measures for sudden adverse developments in patients such as cardiac arrests. Perform preoperative procedures for minor surgery and fill out pre-op checklist. Assist patients in admission, transfer, and perform discharge planning follow-up and documentation. Support the patient and/or family members toward the achievement of treatment plan goals. Provide instructions to the patient on invasive procedures, surgical procedures and post-surgical conditions which were previously provided to the patient by a nurse or physician. Instruct patients on how equipment is used such as oxygen, suction, cardiac monitor, and pulse oximeter. Instruct patients and family on use of prescribed medications, contraindications of medications, and the necessity of proper follow-up care. Provide emotional support to patients and families. Ensure necessary supplies are available and equipment is in functioning order. Provide an orderly, clean, and safe environment for patients and staff. Handle telephone information requests with courtesy, accuracy, and respect for patient confidentiality. Receive information and distribute messages as necessary. Ensure maintenance and re-supply of pharmaceuticals in the clinic. Ensure upkeep and perform checks of emergency equipment i.e., oxygen, emergency cart, suction apparatus, etc. and maintain appropriate logs. Inform the Charge Nurse of any discrepancy on daily checks. Provide care coordination using specific criteria and guidelines as mandated by DOD, BUMED, DHA and TRICARE directives, coordinating with the local MHP clinic, utilization, and Referral Management policies, and obtains additional clinical data as required. Consult, coordinate, and collaborate with MHP team members, referral management team members, specialty care clinics, outside care professionals, agencies, organizations, and other ancillary support systems to assist in the care coordination of MHP patients. Identifies patients in need of care coordination within the MHP teams, through screening patients with a pattern of high risk, unstable and resource intensive utilization of Military Treatment Facility (MTF) or network services such as chronic pain patients. Responsible for coordination of outpatient care services of patients belonging to the MHP clinic area of responsibility that are discharged from other facilities. Follows-up with MHP team members regarding review of patient charts, medical records, and other documents utilized in the treatment and care of MHP patients in need of care coordination. Participate in Virtual health encounters as needed.Minimum Qualifications:* Education: Be a graduate from a Licensed Practical Nurse or Licensed Vocational Nurse program accredited by the Accreditation Commission for Education in Nursing (ACEN) or be a graduate from an accredited program accepted by the applicable State Board of Nursing or a training program accepted by the MTF.* Experience: Possess six months of clinical work experience as a full-time LPN/LVN within the preceding four years.* Licensure: Current, full, active, and unrestricted LPN/LVN license from any state. Prior to commencing services, Licensed Practical (Vocational) nurses shall attend a 2-hour lecture on IV certification. Contractor nurses shall then obtain certifications (e.g., IV, medications and blood administration) in accordance with clinic nursing service and command policy not later than 60 days after commencement of services, including MTF observation of three completed IV sticks.* Life Support Certifications: AHA or ARC BLS Certification * Security:Must possess ability to pass a Government background check/security clearance.The salary range for this position is $25.10/hr. - $44/hr.
