JOB SUMMARY The Clinical Care Navigator coordinates and advocates for the patient's care. This position will facilitate the appropriate use of resources, effective planning, and coordination of services for patients to help ensure they are receiving the appropriate level of care. The Clinical Care Navigator will assist in removing pertinent logistical burdens to ensure patients are afforded time and space to focus on their health. PRIMARY FUNCTIONS Provide healthcare case management to individuals through coordination with the patient, health care providers, and families Compassionate, motivational, and able to relate to different clients with various needs Strong verbal and written communication skills to explain to clients, family members, friends, and professionals the case and care plan Critical thinking and problem solving to determine the best plan of care for each client. Organization to manage several different cases at once Computer literacy to maintain, and manage good case records Conduct intake interview with all new referrals Develop individualized care plans that account for physical, financial, and other constraints and update as needed Provide ongoing planning, implementation, and evaluation of patient's progress Evaluate patient's treatment plan for appropriateness, medical necessity, and cost effectiveness Ability to utilize clinical knowledge to allow the Clinical Care Navigator to discuss patient progress about the current treatment plan with the provider Negotiating and coordinating the delivery of durable medical equipment and/or nursing services Identify patient social determinants of health (SDOH) and refer to SDOH department for any resources he/she may need Ensure that families feel sufficiently equipped and supported Coordinate appropriate follow-up services and appointments to promote wellness Record client's progress including chart referrals, home visits and other notable interactions. Arrange transport as necessary following organizational policy Ability to present in a friendly, positive, and professional demeanor to patients, family, colleagues, and community partners. Meet patients at facility locations to ensure continuity of care and assist in removing logistical barriers Ability to maintain boundaries Travel will be involved between facility locations. This is not a remote position Other projects/duties as assigned. EDUCATION Current Florida LPN License, active and in good standing, preferred Certified Medical Assistant active and in good standing, considered 1+ year's case management experience preferred 2+ years in a customer service focused environment within the healthcare field EXPERIENCE Strong understanding of ethics surrounding the storage of patient information Exceptional leadership, supervision, and motivational skills Committed to promoting patient's well-being Knowledge of/experience working with local social services, public health organizations and resources. Experience in clinical patient navigation services and knowledge of community resources One (1) year minimum experience in Clinical Care Coordination, Care Planning or Chronic Care Management. Computer competency is required One (1) year minimum experience with an EHR (Athena Health preferred) ADDITIONAL QUALIFICATIONS Bilingual a plus (English & Spanish and or Creole). Up-to-date immunizations Valid Driver's License Must be highly organized, detailed, and reliable. Excellent written and oral communication skills Must be a collaborator but able to work independently REPORTING RELATIONSHIP Reports to the Quality Manager PHYSICAL REQUIREMENTS Ability to sit, stand, walk, or view computer screen for extended periods of time Ability to use hands and fingers to handle or feel Ability to reach with hands or arms Ability to perform repetitive hand or wrist motions for extended periods of time Ability to view a computer screen for extended periods of time Ability to perform repetitive hand and wrist motions for extended periods of time Ability to communicate effectively with patients and staff
