The Director of Community Health Strategy is accountable for the development and execution of PacificSource's community health strategies and related programs and partnerships that span key departments: Medicaid Administration, Quality, Population Health, and diversity, equity, and inclusion efforts. This role will support the development and execution of strategies specifically related to social determinants of health (SDOH), health equity, healthcare workforce, and Health-Related Social Needs, among others. The role will focus on building organization and community-wide competencies to address SDOH and equity needs of PacificSource members, and on working with leaders throughout the organization to execute strategies to improve both the company's community presence and impact on Triple Aim outcomes.

Essential Responsibilities:
  • Facilitate the development of PacificSource's framework and plan to implement community health strategies, as informed by organizational strategic priorities.
  • Provide thought leadership and participate directly in execution of the company's strategic plan and major strategic aims.
  • Supervise team members and bodies of work that include SDOH, health equity, healthcare workforce (including Traditional Healthcare Worker initiatives), and Health-Related Social Needs and oversee associated regulatory requirements and deliverables.
  • Be a strategic consultant for creating a Health-Related Social Needs provider network and associated plans to offer technical assistance and creating an oversight and monitoring plan.
  • Advise key strategies to help build and support the healthcare workforce in communities served and advance health equity for members served.
  • Recruit, build, and maintain positive relationships with community-based organizations who may become community health partners by becoming contractors/subcontractors to help further shared priorities.
  • Champion Community Information Exchange efforts to support SDOH strategies.
  • Ensure community health solutions are based on evidence thorough literature review, periodic surveys, and outcomes analyses to assess current offerings and plan for improvements in programs, report out those findings to PacificSource leadership and adjust the strategy and initiatives accordingly.
  • Partner with internal teams to ensure community health strategies are aligned, best practices are shared, and teams are well informed about how to integrate community health priorities in their daily work.
  • Participate with lines of business, sales and marketing leaders in business development activities (e.g. responses to proposals).
  • Facilitate and represent the company in multi-stakeholder boards, steering committees, and work groups to establish common solutions to complex community health problems thus improving equity, community health and coordinated care (community health development).
Supporting Responsibilities:
  • Participation in internal and community based public presentations.
  • Meet department and company performance and attendance expectations.
  • Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.
SUCCESS PROFILE

Work Experience: Minimum of 10 years' experience in healthcare, community health, provider organization, or related area. Relevant experience the following areas: managed care, community health, strategic planning, business development, quality improvement, strategic relationship development, workforce innovation, population health improvement. Experience leading large multi-stakeholder initiatives highly desired. Experience in managing complex projects or project management certification is desirable. Demonstrated ability to develop and execute strategy.

Education, Certificates, Licenses: Bachelor's degree in Public Health, Healthcare Administration, Business, or related field required. Master's degree strongly desired.

Knowledge: Excellent public relations, presentation, and interpersonal skills. Demonstrated successful communication skills, including public presentation, training, meeting facilitation. Maintain high level of knowledge of company products, health reform trends at the Federal and State levels, and the insurance industry. Math skills required including percentages, ratios, graphing. Background in statistics preferred. Demonstrated skills with the following software: Microsoft Word, PowerPoint, Excel, and Visio. Strong analytical and problem-solving skills.
