Description The HARP Care Manager conducts and schedules assessments, referrals, advocacy and supports, counseling, education of patients and enrollees and care team members assuring the patient receives quality services to maintain optimum healthcare needs without barriers. Shift/ Hours : Full-time M-F 8:00am-5:00pm - flexibility as needed. Essential Job Functions: Outreach and engagement to formally enroll referred individuals into the Health Home Care Management services Provide for patients by matching patient needs with appropriate services Conduct assessments, evaluate needs, establish and maintain care plan and maintain referrals for enrollees Become educated on services available in the community and connect client with appropriate agency in the community Provide on-going life skills training to clients to include: money management, shopping, proper nutrition, healthy living, public transportation, etc. Document client visits according to Health Home’s and Credo’s criteria Enroll eligible clients in Health Home Develop and maintain HARP Program; Responsible for tracking all HARP data and reporting Complete HARP eligibility assessments at least annually, and reassessments when there is a significant change in status for HARP members receiving HCBS Maintain caseload of 30-40 clients providing minimum of 30 contacts per month Attend and represent Credo at Single Point Of Entry (SPOE) meetings Assures supports are in place inclusive of peer and family contacts Develops Interim Plan of Care based on preliminary clinical information and assigned level that will identify linkages and services immediately required, based on information received from referral sources if applicable Works with patients’ providers to customize services to best meet members’ needs within the parameters of available services Collaborates with all services providers and establishes team communication plan Monitors goals on a continuing basis and that team is communicating Monitors that care plan is relevant to Credo policies and procedures Provides self-management support Consults with family members and social supports to maintain support consistency Advocates for additional services and linkages as appropriate Maintains current care management documentation and information regarding care management activities within the required EHR system Utilizes supervision: identifies and reports clinical, utilization and outcomes issues. Identifies provider issues and recommendations for improvement. Participates in CQI activities and provider training, and participates in professional development activities Will be required to perform other duties as requested, directed or assigned Note:

Requirements Minimum Job Qualifications: Bachelor's degree in one of the qualifying education fields OR A NYS teacher’s certificate for which a Bachelor’s degree is required OR NYS licensure and registration as a Registered Nurse and a bachelor’s degree OR A Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) AND Two years of experience: • Providing direct services to people with Serious Mental Illness (SMI), developmental disabilities, or substance use orders (SUD); or • Linking individuals with Serious Mental Illness, developmental disabilities, or substance use disorder to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services). AND Support a person centered approach to care AND Must subscribe to a Professional Canon of Ethics. A master’s degree in one of the qualifying education fields may be substituted for one year of experience. AND Supervision from a: • Licensed healthcare professional with prior experience in a behavioral health clinical or care management supervisory capacity; or • Master’s level professional with 3 years’ prior experience supervising clinicians and/or care managers who are providing direct services to individuals with SMI or SUDs. egrees featuring a major or concentration in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field. Other Requirements:

Must maintain a valid Driver’s License, as specified in the personnel policies. Must maintain all laws and policies concerning confidentiality, security, and client rights as specified in the employee handbook, 42 CFR Part 2 and HIPAA. Must maintain current certifications and/or trainings as specified by State law and agency regulations.
