JOB CATEGORY: CARE COORDINATION / CASE MANAGEMENT / SOCIAL WORK / NURSE

POSITION SUMMARY: Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost-effective health outcomes.

Qualifications

REQUIREMENTS

EDUCATION AND/OR EXPERIENCE:
  • Graduate of an accredited nursing program (LPN or RN) AND five year's relevant healthcare experience AND nursing license (e.g., LPN or RN); OR
  • Bachelor's or Master's degree in social work, nursing or other healthcare profession; AND nursing license or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT); AND one year healthcare experience.
Self-starter with strong verbal and written communication skills.
  • Comfort with change and quality improvement processes.
  • Experience with Electronic Health Records strongly preferred.
  • Experience with Motivational Interviewing strongly preferred.
  • Knowledge of New Mexico and local community resources strongly preferred.
  • Bilingual (English / Spanish) or (English / Navajo) preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
  • Must have at time of hire and maintain current throughout employment nursing license or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT).
  • Certified Case Manager (CCM) preferred.
SCREENING REQUIREMENTS:

This position requires successful completion of an initial post-offer of employment:
  • Criminal Records Check.
  • According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
