We are seeking a highly skilled and compassionate Care Coordinator II - Delegated to join our team at Presbyterian Medical Services. As a key member of our healthcare team, you will play a vital role in ensuring our patients receive the highest quality care and support.

Key Responsibilities
  • Work closely with patients, families, and healthcare teams to assess, plan, implement, and evaluate care plans that meet the unique needs of each individual.
  • Develop and maintain strong relationships with community resources and healthcare providers to ensure seamless care coordination.
  • Utilize strong communication and interpersonal skills to effectively engage patients, families, and healthcare teams in the care planning process.
  • Stay up-to-date with industry developments and best practices in care coordination to ensure our patients receive the most effective and efficient care possible.
Requirements
  • Associate's degree and five years of relevant healthcare experience, or Bachelor's degree and four years of relevant healthcare experience, or Master's degree and two years of relevant healthcare experience.
  • Good knowledge of personal computers and software for word processing and data retrieval.
  • Self-starter with strong verbal and written communication skills.
  • Highly organized with excellent follow-through skills.
  • Comfort with change and quality improvement processes.
  • Ability to pursue ongoing education, certification, and/or self-development to remain current with industry standards and business objectives related to care coordination.
Preferred Qualifications
  • Experience with Electronic Health Records.
  • Experience with Motivational Interviewing.
  • Knowledge of New Mexico and local community resources.
  • Bilingual (English / Spanish) or (English / Navajo).
Certifications and Licenses
  • Certified Case Manager (CCM) preferred.
  • First Aid and BLS/CPR certifications within 30 days of hire.
Screening Requirements
  • Successful completion of an initial post-offer of employment Criminal Records Check.
  • Compliance with required testing and immunizations, including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella.
Driving Requirements
  • Valid Driver's License.
  • Current motor vehicle record.
  • Personal owner liability automobile insurance.
  • Compliance with PMS Policies on Automobile Usage and applicable state and federal laws.
