Key Responsibilities- Work closely with patients, families, and healthcare teams to assess, plan, implement, and evaluate care plans that meet the unique needs of each individual.
- Develop and maintain strong relationships with community resources and healthcare providers to ensure seamless care coordination.
- Utilize strong communication and interpersonal skills to effectively engage patients, families, and healthcare teams in the care planning process.
- Stay up-to-date with industry developments and best practices in care coordination to ensure our patients receive the most effective and efficient care possible.
- Associate's degree and five years of relevant healthcare experience, or Bachelor's degree and four years of relevant healthcare experience, or Master's degree and two years of relevant healthcare experience.
- Good knowledge of personal computers and software for word processing and data retrieval.
- Self-starter with strong verbal and written communication skills.
- Highly organized with excellent follow-through skills.
- Comfort with change and quality improvement processes.
- Ability to pursue ongoing education, certification, and/or self-development to remain current with industry standards and business objectives related to care coordination.
- Experience with Electronic Health Records.
- Experience with Motivational Interviewing.
- Knowledge of New Mexico and local community resources.
- Bilingual (English / Spanish) or (English / Navajo).
- Certified Case Manager (CCM) preferred.
- First Aid and BLS/CPR certifications within 30 days of hire.
- Successful completion of an initial post-offer of employment Criminal Records Check.
- Compliance with required testing and immunizations, including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella.
- Valid Driver's License.
- Current motor vehicle record.
- Personal owner liability automobile insurance.
- Compliance with PMS Policies on Automobile Usage and applicable state and federal laws.
