Job Summary

Presbyterian Medical Services is seeking a highly skilled Care Coordinator II - Delegated to join our team. As a key member of our Interdisciplinary Care Team, you will work closely with patients, families, and healthcare professionals to provide comprehensive care coordination services.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to ensure optimal outcomes
  • Collaborate with healthcare providers to coordinate care and services
  • Provide education and support to patients and families to promote self-management and independence
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
Requirements
  • Associate's degree and 5 years of relevant healthcare experience, or Bachelor's degree and 4 years of relevant healthcare experience, or Master's degree and 2 years of relevant healthcare experience
  • Good knowledge of personal computers and software for word processing and data retrieval
  • Strong verbal and written communication skills, with ability to work effectively in a team environment
  • Highly organized with excellent follow-through skills, and ability to adapt to changing priorities and deadlines
Preferred Qualifications
  • Experience with Electronic Health Records
  • Knowledge of New Mexico and local community resources
  • Bilingual (English/Spanish) or (English/Navajo)
